右美托咪定复合芬太尼用于患者自控镇痛的临床观察被引量：47

摘要目的:观察右美托咪定复合芬太尼用于患者静脉自控镇痛是否改善镇痛效果和减轻与芬太尼有关的副作用。方法:全组60例女性患者,年龄25～60岁,ASAⅠ或Ⅱ级,全麻下行妇科手术。随机双盲分为两组(n=30):F组—镇痛泵100mL生理盐水中+芬太尼0.8mg(8μg/mL);FD组—镇痛泵100mL生理盐水中+芬太尼0.8mg+右美托咪定400μg(4μg/mL)。观察记录:首次PCA时间;PCA按压总次数和无效次数;24h芬太尼总用量;不同时段疼痛强度;血流动力学和呼吸的变化。结果:FD组患者术后24h内芬太尼用量明显少于F组[(440±19.6)μgvs(576±25.5)μg,P<0.01]。术后2、4和8h,FD组及MAP及HR明显较术前减少或降低(均P<0.01)。FD组在术后0～24h内恶心和瘙痒发生率显著低于F组(13%vs40%和17%vs40%,P<0.05);未发现心动过缓、血压过低、过度镇静和呼吸抑制现象。结论:右美托咪定复合芬太尼用于患者静脉自控镇痛可改善芬太尼镇痛效果,节俭芬太尼用量、减少有关副作用;提高静脉芬太尼患者自控镇痛的满意度。
