急性混合细胞白血病44例的临床诊治分析 被引量：8

摘要 目的:分析急性混合细胞白血病(biphenotypic acute leukemia,HAL)的临床和实验室特征以及治疗和预后。方法:根据骨髓细胞形态学、免疫表型、细胞遗传学和分子生物学结果诊断HAL,评价疗效及影响疗效的相关因素。结果:44例HAL临床表现可有发热、淋巴结肿大、脾大、肝大、睾丸浸润等,所有患者骨髓增生明显活跃,近半数患者属高白细胞白血病。进行免疫表型检测者44例,其中B/M双表型33例、T/M双表型10例,B/T型1例。诱导化疗的总体完全缓解(CR)率为32%,34例接受治疗后11例获得CR,有2例巩固强化治疗后持续缓解,中位总生存期(OS)为13个月(3～43个月),1例仅给予淋系和2例仅给予粒系的诱导未达到CR。结论:HAL患者存在着发病年龄偏大,高白细胞、预后不良型染色体改变比例较高。治疗上给予兼顾粒、淋二系的诱导和巩固化疗方案治疗,缓解率高及总生存期长。但长期疗效的评价还有待进一步观察。
