摘要
目的探讨超声弹性成像(RTE)在肝纤维化分期中的应用价值。方法选择不同病理分期的肝纤维化患者224例,应用RTE评分标准将患者分级,对比其与病理检查分期的相关性,并界定肝纤维化与肝硬化的RTE评级的分界点。结果肝脏RTE分级为F0、F1、F2级的患者,病理分期多为纤维化(S0、S1、S2、S3)(P<0.05);RTE分级为F4级的患者,病理分期多为肝硬化(S4)(P<0.05);RTE分级为F3时,病理分期分布差异无统计学意义(P>0.05)。RTE评分方法与病理分期方法在评价肝纤维化程度时有较高的一致性(Kappa值=0.816)。结论 RTE可作为较客观地评价肝纤维化分期的辅助检查方法;此外,F3级可认为是肝纤维化与肝硬化分界点。
Objective To investigate the application value of ultrasonic real-time tissue elastography(RTE) score in liver fibrosis graduation. Methods Two hundred and twenty-four patients with hepatic fibrosis in different pathological stages were enrolled in this study, they were examined by RTE, then they were classfied according to ultrasonic elastography score criteria, the correlation between pathological graduation and elastography graduation was compared. Results The patients with F0, F1 and F2 by ultrasonic elastography score were mainly distributed within the liver fibrosis (SO, S1, S2, S3 ) , while patients with F4 mainly within the cirrhosis (S4). There was no significant difference of uhrasonic elastography score F3 between the distribution of liver fibrosis and cirrhosis (P〉0.05). There was a high consistency between ultrasonic elastography score and pathological staging ( Kappa value = 0. 816 ). Conclusion Ultrasonic elastography technique can be used as an objeetive examination method in grading liver fibrosis. Besides, ultrasonic elastography score F3 is the watershed between liver fibrosis and cirrhosis.
