绝经后女性类风湿关节炎患者骨密度与生化免疫指标相关性的研究 被引量:1

Correlation of Bone Mineral Density and Biochemico-immunological Markers in Postmenopausal Female Patients with Rheumatoid Arthritis

摘要 目的寻找血生化免疫指标中与类风湿关节炎(RA)患者骨密度(BMD)相关的参数。方法选择32例绝经后女性RA患者,测定其血沉(ESR)、C反应蛋白(CRP)、类风湿因子(RF)、纤维蛋白原(Fib)、血小板(PLT)、血红蛋白(HB)、碱性磷酸酶(ALP)、钠(Na+)、钙(Ca2+)、免疫球蛋白(IgG,IgA,IgM)及补体(C3,C4)值;采用双能X射线BMD检测仪测量患者的腰椎(L1~L4)及髋部BMD。结果绝经后女性RA患者腰椎和髋部BMD与年龄呈显著负相关(腰椎r=-0.505,P〈0.01;右髋部r=-0.424,P〈0.05;左髋部r=-0.487,P〈0.01);多元线性回归分析显示:髋部BMD与ALP、RF及Na+关系密切,前2者与BMD呈负相关,后者与BMD呈正相关。结论绝经后女性RA患者的BMD随年龄增长呈下降趋势,ALP、RF及Na+与髋部BMD关系密切。 Objective To investigate the correlation between the level of serum biochemico-immunological markers and bone mineral density(BMD) in postmenopausal female patients with rheumatoid arthritis(RA).Methods 32 postmenopausal female patients with RA hospitalized in our wards from June 2011 to November 2011 were selected,and erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF),fibrinogen(Fib),platelet(PLT),hemoglobin(HB),serum alkaline phosphatase(serum-ALP),Na+,Ca2+,immunoglobulin(IgG,IgA,IgM) and complement(C3,C4) were determined,and BMD of the lumbar spine(L1-L4) and hip was measured by dual energy X-ray absorptiometry(DEXA).Results BMD of the lumbar spine and hip in postmenopausal female patients with RA was significantly negatively correlated with age(lumber r =-0.505,P 0.01;right hip r =-0.424,P 0.05;left hip r =-0.487,P 0.01);multiple linear regression analysis showed there was a strong linear relation between hip BMD and ALP,RF,Na+.ALP and RF were negatively correlated with BMD,and Na+ was positively correlated with BMD.Conclusion BMD in postmenopausal female patients with RA is decreased with aging,and there is a strong correlation between hip BMD and ALP,RF,Na+.
