摘要
目的探讨闭合复位经皮外侧髁部穿针固定治疗GartⅠandⅡ、Ⅲ型儿童肱骨髁上骨折的临床效果。方法本组23例儿童肱骨髁上骨折(GartlandⅡ型9例,Ⅲ型14例),闭合复位经皮外侧髁部穿克氏针加屈肘长臂石膏托外固定3周。3周后去石膏锻炼,4-6周拔去固定的克氏针。结果随访6-24个月,按Flynm评定标准,优17例(73.9%),良5例(21.7%),一般2例(4.4%),优良率95.6%。结论闭合复位经皮外侧髁部穿针固定治疗儿童肱骨髁上骨折,具有复位良好、创伤小、并发症少、固定可靠等优点,疗效满意,值得临床推广。
Objective To explore the therapy effect of supracondylar fracture（Gartland type Ⅱ and Ⅲ） in children with closed reduction and internal fixation with percutaneous crossed-pin from outboard by using C-arm image intensifier.Methods All of fractures（23 cases： type Ⅱ 9 cases,type Ⅲ 14 cases） were treated by closed reduction and internal fixation with percutaneous crossed-pin from outboard by using C-arm X-ray perspective under the chloramine vena anaesthesia.Arter operition,keeping outer fixation in gesso with bending elbow for three weeks.After three weeks,blackout gesso and do exercies,After 4 or 6 weeks,pull out pins of internal fixation.Results All cases were followed-up from 6-24 months.To asess the elbow function according to Flynm criterion：17 excellent（73.9%）,5 good（21.7%）,2 common（4.4%）.The good and excellent rate were 95.6%.Conclusion Closed reduction and percutaneous pinning of supracondylar fractures from outboard of the humerus in children by using C-arm X-ray perspective is a safe,effective method with minimal damage and has good curative effect.The way is worth of spreading out in clinic.
