摘要 [目的]探讨持续封闭负压引流(vacuum-sealing drainage,VSD)技术治疗创伤骨科中四肢皮肤及软组织严重创伤的临床效果.[方法]对2007年9月至2011年09月本院临床所收治的30例复杂的创伤性皮肤软组织严重缺损患者行手术清创后应用VSD敷料覆盖创面,负压吸引器持续吸引治疗7~21 d后,拆除敷料行二期植皮或皮瓣转移术.[结果]30例患者中5例患者一次使用VSD治疗后行二期植皮或皮瓣转移术全部成活;25例患者由于全身多处严重软组织创伤行多次VSD治疗后植皮全部成活.[结论]VSD技术能有效地清除创面的分泌物和坏死组织,刺激肉芽生长,减少创面感染明显缩短创伤性皮肤软组织缺损的治疗时间. [Objective]To explore the clinical efficacy of continuous vacuum-sealing drainage(VSD) tech nology for the treatment of severe traumatic skin and soft tissue defect in department of orthopedics and trauma. [Methods] Thirty patients with complicated traumatic skin and soft tissue defect in our hospital from Sep. 2007 to Sep. 2011 were enrolled in this study. VSD dressing was used to cover the wound after surgical debridement. Suction was taken continuously in the following 7 to 21 days, then the dressing was removed and secondary dermepenthesis or the transfer of skin flap was performed. [Results] All the grafted skin in 5 of 30 patients undergoing secondary dermepenthesis or the transfer of skin flap after VSD treatment survived. All the grafted skin of other 25 patients with systematic multiple severe soft tissue trauma undergoing many VSD treatment survived. [Conclusion] VSD technology can effectively remove the secretions and necrotic tissue on the wound surface, stimulating granulation growth, decrease the wound infection and obviously shorten the treatment time of traumatic skin and soft tissue defect.
