摘要
目的探讨MRI扩散加权成像(diffusion weighted imaging,DWI)及表观扩散系数(apparent diffusion coef-ficient,ADC)在鉴别良、恶性颈部淋巴结病变中的诊断价值。方法颈部淋巴结肿大患者62例,其中恶性淋巴结病变48例,良性淋巴结病变14例。62例患者均行常规MRI检查(T1WI、T2WI)及SE-EPI DWI扫描(b值分别为:500、800、1 000mm2.s-1),同时进行ADC的测量并计算淋巴结的平均ADC值。结果良、恶性淋巴结在MRI DWI上均表现为高信号。与常规MRI扫描序列(T1WI、T2WI)对比,在DWI图像上淋巴结会发生一定程度的变形,且随着b值的增加,变形越明显。恶性淋巴结的ADC值为(0.827±0.431)×10-3 mm2.s-1,良性淋巴结的ADC值为(1.225±0.283)×10-3 mm2.s-1,二者比较差异有统计学意义(P<0.01)。结论 MRI DWI对鉴别良、恶性颈部淋巴结病变的诊断价值较常规MRI更高,其ADC值可以定量分析良、恶性淋巴结病变。
Objective To explore the value of diffusion-weighted imaging (DWI) and apparent diffusion coefficient(ADC) in the differential diagnosis of benign and malignant neck lymph node lesions. Methods Conventional MR1 (T1WI and T2WI) and SE-EPI DWI (b = 500,800 and 1 000 mm^2·s^-1) were performed in 62 patients with cervical lymphadenectasis(48 patients with malignant lymph node lesions and 14 patients with benign lymph node lesions). At the same time, ADC values were measured and mean ADC values were calculated. Results Both benign and malignant lymph nodes showed high signal intensity on DWI. Compared with conventional MRI sequences (T1WI and T2WI) ,DWI resulted in image distortion and the distortion became more significant with increased b values. There were significant differences in mean ADC values between malignant lymph nodes and benign lymph nodes [ (0.827±0.431)×10^-3mm^2·s^-1 vs ( 1.225±0.283)×10^-3mm^2·s^-1 ,P〈0. 01]. Conclusion DWI has a higher value for differential diagnosis of benign and malignant neck lymph lesions than conventional MRI. The ADC values contributes to quantitative analysis of benign and malignant lymph lesions.
