摘要 目的初步探讨HIV阳性患者CD4+T淋巴细胞计数与牙龈出血指数的关系。方法随机选择符合纳入标准的受试者79名。检查受试者菌斑指数、出血指数和血液CD4+T淋巴细胞计数。结果 CD4+T淋巴细胞计数<200个/mm^3者中,54.05%出血指数为1~2;200~500个/mm^3者中,57.58%出血指数为2~4;>500个/mm^3者中,55.56%出血指数为0~1。总体来看,CD4+T淋巴细胞计数与HIV感染者牙龈出血指数无相关关系(P>0.05);CD4+T淋巴细胞计数分层后,并控制菌斑指数进行偏相关分析,CD4+T淋巴细胞计数<200个/mm^3和200~500个/mm^3者偏相关系数分别0.612(P=0.000)、0.353(P=0.048),CD4+T淋巴细胞计数和出血指数有相关性,2组出血指数分别是1.410±0.791和1.904±1.201,差异有显著性(P<0.05)。结论 CD4+T淋巴细胞计数<500个/mm^3的HIV感染者的牙龈出血指数和CD4+T淋巴细胞计数有相关性,且中度免疫抑制者牙龈出血指数高于重度免疫抑制者。而CD4+T淋巴细胞计数>500个/mm^3的HIV感染者的牙龈出血指数和CD4+T淋巴细胞计数没有相关性。 Objective To investigate the correlation of CD4 + T lymphocyte count and gingival bleeding index in HIV-positive patients.Methods Seventy-nine HIV-positive patients were selected according to inclusion criteria.Periodontal examination included plaque index(PLI) and gingival bleeding index(BI),and CD4 + T lymphocytes were counted.Results The BIs were 1-2 in 54.05%of patients with CD4200cells/mm^3,24 in 57.58%of patients with CD4 between 200-500cells/mm^3,0-1 in 55.56%of patients with500cells /mm^3 respectively.No correlation was found between CD4 cell counts and BI when all patients were evaluated.Positive correlations were observed between CD4 cell counts and BI for the subpopulations with CD4200cells/mm^3(r = 0.612,P = 0.000) and between 200—500 cells/ mm^3(r=0.353,P =0.048).The BIs in these two subpopulations were 1.410±0.791 and 1.904±1.201 respectively.The difference was statistically significant(P0.05 ).Conclusion Patients with CD4 cell counts500cells /mm^3 showed positive association between CD4 cell count and BI,and severely immunocompromised HIV-positive patients showed less severe ginginval inflammation than moderate ones.Patients with CD4 cell counts500cells /mm^3 showed no association between CD4 cell count and BI.
