超声和^(99m)TC-tetrofosmin显像在鉴别乳腺肿块良、恶性中的价值

摘要 目的:探讨超声和99mTC-tetrofosmin显像在鉴别乳腺肿块良恶性中的价值。方法:选取我院行乳腺肿块切除术,术前均先后经超声和99mTC-tetrofosmin显像的64例病例,其中恶性34例,良性30例,进行对比分析。结果:超声诊断乳腺癌(真阳性)30例,假阴性4例,假阳性6例,真阴性24例,其敏感性为88.24%,特异性为80.00%,准确性为84.38%,阳性预告值为83.33%,阴性预告值为85.71%;99mTC-tetrofosmin确诊乳腺癌(真阳性)29例,假阴性5例,假阳性7例,真阴性23例,其敏感性85.29%,特异性为76.67%,准确性为81.25%,阳性预告值为80.56%,阴性预告值为82.14%。结论:超声与99mTC-tetrofosmin在鉴别乳腺肿块良恶性中各有优缺点,二者结合可提高乳腺癌诊断的准确性。 Objective:To evaluate the significance of the differential diagnosis of uhrasonographic and 99-TC-tetrofosmin in breast tumor. Methods: In 64 cases with breast lesions underwent both ultrasonography and 99m-TC-tetrofosmin, all patients had histopathological confirmation of the nature of the breast lesions, of them 34 were malignant and 30 were benign. Results: Uhrasonography was positive in 36 patients(30 true positive, 6 false positive)and negative in 28 patients(24 true negative, 4 false negative) the corresponding values of 99m TC-tetrofosmin were 36 (29,7) and 28 (23,5). The sensitivity of ultrosonography was 88. 24%, the specificity was 80. 00%, the accuracy was 84. 38% and the positive and negative predictive value were 83. 33% and 85.71% The corresponding values for 99-TC-tetrofosmin were 85. 29 %, 76. 67 %, 81.25 %, 80. 56 %, 82. 14 %. Conclusion: Utrasonography are still the commonly used methods to diagnose breast lumps, because of their low costs and fairly accurate results,When difficulties are met with ultrasonography, 99=TC-tetmfosmin may give some help.
