CTA evaluation of median arcuate ligament compressing the celiac axis

摘要 目的:探讨正中弓状韧带(Median arcuate ligament,MAL)压迫腹腔干的电子计算机断层血管造影(Computed to-mography angiography,CTA)表现及正中弓状韧带综合征(Median arcuate ligament syndrome,MALS)的诊断标准。方法:回顾性分析吸气末屏气扫描腹部CTA的病例988例。其中前上方外压性狭窄9例,1例经手术证实为MALS。设计腹部CTA吸气末和呼气末屏气扫描检查50例。结果:MAL压迫腹腔干CTA表现:腹腔干起始部前上方"V"型凹陷,重度狭窄为典型"钩状"形态;显示带状膈肌脚结构及其下缘纤维对管腔的压迫;呼气末压迫程度可加重,吸气末减轻或消失。结论:腹部CTA在MAL压迫腹腔干和MALS的诊断和鉴别诊断中有较大的临床应用价值。
