摘要
目的探讨中药抗癌消水膏外敷治疗老年恶性胸腔积液的临床疗效和特点。方法选择体力状况(PS)〉2的27例老年非小细胞肺癌并发恶性胸水者,用抗癌消水膏外敷患者的胸壁,观察评估患者的胸水、胸痛、呼吸困难、生活质量等;用微球流式芯片技术检测胸水中细胞因子IL-2、IFN-γ、IL-4、IL-10的水平。根据胸水改善是否改善将患者分为两组:胸水改善组和胸水未改善组。结果胸水完全缓解有4例,部分缓解9例,无变化14例,总有效率为48.1%;胸痛改善15例,改善率为55.6%;呼吸困难改善18例,改善率为66.7%;生活质量改善17例,稳定6例,恶化4例,改善率为62.96%,胸水改善组治疗后IL-2和IFN-γ上升,IL-4和IL-10下降,与治疗前比较差异具有显著统计学意义(P〈0.05);胸水未改善组治疗前后各细胞因子水平差异无统计学意义(P〉O.05)。结论抗癌消水膏外敷治疗老年非小细胞肺癌恶性胸水能改善胸痛、呼吸困难症状和提高生活质量,是一种有效的方法;胸水中Th1细胞因子优势化可能是影响抗癌消水膏疗效的主要因素之一。
Objective To study the clinical effect of Anticancer Water Removing Cream on malignant pleural ef- fusion in the elderly patients. Methods 27 elderly patients whose physical strength were more than 2 with non-small cell lung cancer complicated by malignant pleural effusion weretreated with Anticancer Water Removing Cream in their chests. The improved situation of hydrothorax, chest pain, dyspnoea and life quality were observed. The levels of cytokines (IL-2, IFN-γ, IL-4 and IL-10) in hydrothorax were detected by microballoon streaming chip technique. The changes of these cytokines in valid group and invalid group before-and-after treatment with Anticancer Water Removing Cream were compared. Results The total cure rate of Anticancer Water Re- moving Cream on malignant hydrothorax was 48.1%. The symptoms of dyspnoea, chest pain, and quality of life were sig- nificantly improved. The levels of IL-2 and IFN-γ were elevated, whereas the levels of IL-4 and IL-10 were reduced in the valid group. Conclusions Anticancer Water Removing Cream application can relieve symptoms of chest pain, dyspnoea, and improve the quality of life of the elderly patients with non-small cell lung cancer complicated by malignant pleural effusion, and is an effective and safe treatment. Superiority of cytokines in Thl cells of hydrothorax is one of the main factors to influence the curative effect.
