摘要
目的观察老年高血压伴左室肥厚(hypertension with left ventricular hypertrophy,HLVH)患者24h动态血压波动的特点,制定相关治疗措施。方法 76例老年高血压患者根据心脏多普勒超声结果分为高血压伴左室肥厚组(HLVH)35例、单纯高血压组(NHLVH)41例,采用动态血压监测系统对比分析两组患者24h血压变化及波动的相关参数。结果 HLVH组24h平均收缩压、日平均收缩压、夜平均收缩压、夜平均舒缩压、夜间舒张压负荷与NHLVH组比较,差异有统计学意义(P<0.05);日间收缩压负荷、夜间收缩压负荷及24h平均脉压与NHLVH组比较,差异有统计学意义(P<0.01);血压昼夜节律构成比例与NHLVH组比较差异也有统计学意义(P<0.05)。结论了解老年高血压合并左室肥厚患者动态血压变化及波动的特点将有助于对老年患者进行针对性诊治。
Objective To investigate the details about the 24-hour blood pressure fluctuation in senile hypertension with left ventricular hypertrophy.Methods Seventy six senile patients with hypertension were divided into two groups according the result of echocardiography：senile hypertension with left ventricular hypertrophy（HLVH,n=35）,single senile hypertension（NLVH,n=41）.The fluctuating feature of the 24-hour blood pressure in two groups were compared by 24-hour ambulatory blood pressure monitoring（ABPM）.Results In patients with left ventricular hypertrophy,the 24-hour mean systolic blood pressure（mSBP）,the day mSBP,the night mSBP,the night mean diastolic blood pressure（nmDBP）,the night DBP load（nDBPL）,the day SBP load（dSBPL）,the night SBP load（nSBPL）and 24-hour mean pulse pressure（mPP） were significantly higher compared with the single senile hypertension（P0.05）,the proportion of the non-dipper hypertension was of the same case（P0.05）.Conclusion The study of the details about the 24-hour blood pressurefluctuation can help us to carry out the related treatment measure in senile hypertension with left ventricular hypertrophy.
