摘要
目的:观察踝足矫形器(AFO)对痉挛型脑瘫患儿运动功能及ADL的影响。方法:CP患儿26例,随机分为AFO组和对照组各13例,在采用相同的综合康复治疗的基础上,AFO组患儿每次治疗后坚持佩戴AFO。治疗前后采用改良Ashworth评定2组患儿腓肠肌痉挛程度;测量休息位踝关节角度(rROM);脑瘫粗大运动评分量表(GMFM)评定站立和走跑跳两大运动功能及参照Barthel指数评定ADL能力。结果:经过3个月的治疗,2组患儿腓肠肌痉挛程度及rROM均明显改善(P<0.05);AFO组表现更明显(P<0.05)。GMFM和ADL评分AFO组高于治疗前及对照组(P<0.05),对照组治疗前后变化不明显。结论:AFO可降低CP患儿下肢的肌张力,矫正足踝部畸形。
Objective: To observe the effectiveness of ankle foot orthoses (AFO) for motor function and ADL in children with spastic cerebral palsy. Methods: The 26 patients meeting the inclusion criteria of cerebral palsy with spastic quadriplegia were randomly divided into two groups: AFO and control. On the basis of the same comprehen- sive rehabilitation intervention, the children in AFO group wore AFO for three months. The muscle tension, ankle range of motion, GMFM and ADL were evaluated by using the modified Ashworth before and after treatment. Results: After treatment for 30 months, the muscle tension, ankle range of motion, GMFM and ADL were improved as compared with those before treatment in both groups to varying degrees (P〈0. 05), especially in AFO group (P〈0.05). Conclusion: AFO in children with spastic cerebral palsy can reduce lower limb muscle tension, and correct foot and ankle deformities.
