原发性醛固酮增多症患者的血清肿瘤坏死因子α和白细胞介素6水平 被引量:2

Serum tumor necrosis factor α and interleukin-6 levels in patients with primary aldosteronism

摘要 目的观察原发性醛固酮增多症(原醛)患者中血清肿瘤坏死因子α(TNF-α)和白细胞介素6(IL-6)的水平,并分析其与肾素血管紧张素醛固酮系统的相关性。方法收集新疆维吾尔自治区人民医院高血压科的原发性高血压(EH)患者67例作为对照组,原醛患者93例为病例组,采用放射免疫法检测两组患者TNF-α和白细胞介素6(IL-6)的水平,采用免疫比浊法检测血清高敏C反应蛋白(hsCRP)的浓度,比较两组间TNF-α、IL-6和hs-CRP的水平差异。结果原醛组患者总胆固醇水平较EH组患者低(P<0.01),而收缩压较EH组患者高(P<0.01);协方差校正总胆固醇和收缩压后,原醛组患者的IL-6水平[(0.18±0.05)比(0.24±0.05)μg/L,P<0.01]和hsCRP浓度[ln100hsCRP(3.79±1.3)比(4.39±0.94)mg/L,P<0.01]均较EH组患者低,而两组间TNF-α的水平无统计学差异;Pearson相关分析显示在所有患者中IL-6水平与血清醛固酮水平和醛固酮肾素活性比值(ARR)呈负相关(r分别为-0.183、-0.331,P<0.01)。结论原醛患者血清IL-6的水平较EH患者低;在所有患者中血清IL-6水平与血清醛固酮水平和ARR呈负相关。 Objective To investigate the levels of tumor necrosis factor alpha (TNF-a) and interleukin-6 (IL-6) in patients with primary aldosteronism (PA) and analyze its association with the renin-angiotensin-aldosterone system. Methods One hundred sixty patients(including 93 PA patients and 67 EH patients)were recruited from hyperten- sion department of People's Hospital of Xinjiang Uygur Autonomous Region. The concentration of TNF-α and IL-6 were measured with radioimmunoassay, and the concentration of high-sensitivity C-reactive protein (hsCRP) were measured with immunoturbidimetry. Results Compared with EH patients, PA patients had lower serum total cho- lesterol concentration (P〈0.01) and higher systolic blood pressure level (P〈0.01). After correcting the serum total cholesterol and systolic blood pressure, PA patients had lower levels of IL-6 [-( 0.18 ± 0.05 ) vs ( 0.24 ± 0.05)μg/L, P〈0.01] and hsCRP[ln100 hsCRP(3.79±1.3) vs (4.39±0.94)mg/L,P〈0.01] than EH patients, but there was no significant difference in the level of TNF-α between two groups(P〉0.05). IL-6 presented a remarkable correlation with serum aldosterone levels (r= -0.183, P〈0.01) and aldosterone/plasma renin activity ratio (ARR) (r=-0.331, P〈0.01) in all patients. Conclusion The present study shows that serum IL-6 levels are lower in patients with PA than in patients with EH. There is a significant negative association between IL-6 and serum aldosterone, ARR in all patients.
