摘要 目的了解分化型甲状腺癌(differentiated thyroid carcinoma,DTC)患者术后随访过程中,在不同促甲状腺激素(thyroid-stimulating hormone,TSH)状态下检测甲状腺球蛋白(thyroglobulin,TG)对于肿瘤复发或转移预测的意义。方法回顾性分析100例接受手术+131I治疗的DTC患者,其中TSH抑制组50例(A组),TSH刺激组50例(B组)。根据其是否转移或复发分为阳性组及阴性组,分别测定两组患者血清TG水平。结果 A组中阳性患者的血清TG水平为(15.35±19.42)μg/L,阴性患者的血清TG水平为(1.12±1.34)μg/L,差异有统计学意义(P=0.001)。B组中阳性患者的血清TG水平为(56.14±79.28)μg/L,阴性患者的血清TG水平为(20.02±33.76)μg/L,差异有统计学意义(P=0.001)。两组间复查阳性患者及复查阴性患者间血清TG水平的差异均无统计学意义(P>0.05)。结论血清TG水平在TSH抑制或刺激情况下对于监测甲状腺癌的复发和转移均具有一定的指导意义。 Objective To discuss the prediction significance for tumor recurrence or metastasis for testing thyroid globulin(state thyroglobulin,TG) in different state of thyroid-stimulating hormone(TSH) during the follow-up period of differentiated thyroid carcinoma(DTC) patients.Methods A retrospective analysis was done in 100 cases of DTC patients who received surgical resection and 131I radioiodine therapy.All cases were divided into TSH inhibit group of 50 cases(group A) and TSH stimulate group of 50 cases(group B).Determination of two groups of patients serum TG level,according to whether the recurrence or metastasis is divided into positive and negative groups.Results The serum TG values were(15.35±19.42) μg/L for positive patients and(1.12±1.34) μg/L for negative patients in group A.The difference was statistically significant(P=0.001).The serum TG values were(56.14±79.28) μg/L for positive patients and(20.02±33.76) μg/L for negative patients in group B.The difference was statistically significant(P=0.001).There were no significant differences between the group A and group B(P0.05).Conclusion There was an important directive significance for predicting the recurrence and metastasis of DTC in monitoring serum TG level,whether the TSH levels was suppressed or stimulated.
