摘要
目的观察活血化瘀方药瘀毒清治疗真性红细胞增多症(PV)的临床疗效。方法对20例被确诊为PV的患者均予瘀毒清9 g口服,3次/d,连服3个月,观察治疗前后症状体征积分、血瘀证积分、血常规、骨髓涂片、骨髓活检、肝脾肿大以及心肝肾功能变化情况。结果患者的临床症状和体征均得到改善,症状、体征积分由(14.00±1.33)分降至(5.05±1.30)分,贫血和肝脾肿大的变化不明显;血瘀证积分由(56.60±4.50)分降至(30.40±4.12)分;红细胞计数、血红蛋白、白细胞计数和血小板降至正常(P均<0.05);骨髓涂片显示,骨髓增生明显活跃或极度活跃的病例减少(P<0.01);PV有效率为80%。结论瘀毒清治疗PV可以改善患者血瘀证候,改善骨髓增生情况,减轻临床症状,提高生活质量,疗效确切,但对肝脾肿大变化的影响不明显。
Objective It is to observe the therapeutic efficacy of Yuduqing which can activate blood circulation to dissipate blood stasis on polycythemia vera （ PV）. Methods Twenty patients with PV were given Yuduqing 9 g by oral use three times a day and for three months. Symptom and sign scores, blood stasis scores,blood routine test,bone marrow smear,bone marrow biopsy, sizes of liver and spleen,and the function of liver and kidney were evaluated before and after treatment. Results The patients＇symptoms and signs were all improved. The symptom and sign scores were reduced from 14.00 ±1.33 to 5.05 ±1.30 respectively, blood stasis scores were reduced from 55.60±4.50 to 30.40±4.12 respectively. In blood routine test, the abnormally elevated RBC, Hb, WBC, and platelet counts decreased to normal;bone marrow smear showed that the number of normal bone marrow increased and that of abnormal bone marrow decreased;there was no significant difference in myelofibrosis and splenomegaly; the effective rate for PV was 80%. Conclusion Yuduqing could alleviate the blood stasis symptoms, improve the quality of life and the bone marrow function and make the hyperactive bone marrow function to normal in most of the PV patients, its curative effect on PV is confirmed, but it has no significant effects on hepatosplenomegaly.
