摘要
目的 探讨急性单纯性阑尾炎患者血清可溶性细胞间粘附分子-1（ sICAM-1）和可溶性P-选择素（sP-sel）水平变化及临床价值.方法采用保守药物治疗98例急性单纯性阑尾炎患者,于治疗前后检测患者血液中sICAM-1和sP-sel水平.结果 ①临床治疗总有效率达94.10％.②急性单纯性阑尾炎患者治疗前血清sICAM-1和sP-sel水平均显著高于对照组（P＜0.01）；治疗后血清sICAM-1和sP-sel水平均显著低于治疗前（P＜0.01）,但仍显著高于正常对照组（P＜0.01）.③血清sICAM-1与sP-sel呈显著正相关（r=0.518,P＜0.01）.结论 急性单纯性阑尾炎患者存在血清sICAM-1和sP-sel水平异常表达,保守药物治疗可明显改善临床症状,且能够显著降低血清slCAM-1和sP-sel水平,测定血清sICAM-1和sP-sel水平可作为病情判定和疗效评定的一个良好指标.
Objective To discuss the change of serum soluble intereellular adhesion molecule-1 （ slCAM-1 ） and soluble P-selee- lin（sP-sel） levels and their clinical value in patients with acute simple appendieitis. Methods Ninty-eighl patients with aeule simple appen- dieilis were cured by conservative medicine,aud their serums ICAM-1 anti sP-sel lewds win〉 deleeled hefln＇e and after the treatment. Results ~The total eft〉etive rate of clinical trealmenl was 94.10%. （,_~Serum ICAM-1 and sP-sel levels of the patients hefore treatment were signif- icanlly higher than that of the control groups（ P 〈 0.01 ） ; .~there was positive eorrelalion hetween the serum ICAM-I and sP-sel levels（ r = 0. 518,P 〈0.01 ）. Conclusion The expression level of serums ICAM-I and sP-sel was abnormal in patients with acute simple appendicitis. The vonservative medicine treatment can obviously improve clinical symptom,and significantly decrease serum levels of ICAM-1 and sP-sel. Detecting serum level of ICAM-I and sP-sel are valuahle for evaluation of patient＇s, condition and curative effect.
