摘要
目的:探讨单双极射频椎间盘髓核成形术治疗颈椎间盘突出症的临床疗效及安全性。方法:回顾2007年1月至2011年1月的颈椎间盘突出症住院患者105例(共181个椎间盘),在CT引导下行射频椎间盘髓核成形术,其中单极射频治疗49例,双极射频治疗56例。观察术前和术后3、7 dVAS评分、术后30、90、180 d的MacNab疗效评价,同时记录两组患者的住院时间和并发症。结果:双极组治疗后各随访时间点的疗效均明显高于单极组(P<0.05),双极组患者术后短期的VAS评分明显低于单极组(P<0.05),两组均未发生严重并发症。结论:双极射频椎间盘髓核成形术是一种安全、有效的治疗颈椎间盘突出的微创手术方法。
Objective： To compare efficacy and safety between bipolar percutaneous intradiscal radiofreque-ncy thermocoagulation（PIRFT） and monopole PIRFT in treating cervical intervertebral disc protrusion patients.Methods： A total of 105 patients（181 intervertebral discs） was treated by PIRFT,of which 49 cases were treated by monopole PIRFT and 56 cases by bipolar PIRFT.The patients were followed-up on the 3th,7th after treatment,according to the VAS score;and on the 30th,90th and 180th day after treatment,according to Modified MacNab.Results： According to Modified Macnab and VAS score,the effect of bipolar PIRFT was significantly better than that of monopole PIRFT（P 0.05）.During six-month follow-up,no serious complications appeared in this study.Conclusion： Bipolar PIRFT is a safe and effective minimally invasive treatment for cervical intervertebral disc protrusion.
