摘要
目的 探讨鼻软骨支架重组联合鼻中隔软骨移植整复成人单侧唇裂鼻畸形的方法与效果.方法 开放式鼻成形术切口下,分离鼻中隔软骨与鼻上外侧软骨的连接部,切取部分鼻中隔软骨.在中线位重新固定鼻中隔尾端,以患侧鼻上外侧软骨为支点,斜向缝合鼻中隔软骨以矫正鼻中隔偏斜.重新对位缝合两侧鼻翼软骨穹窿部,并用鼻中隔软骨移植以加强鼻小柱及鼻尖支撑.悬吊缝合内收并上提患侧鼻翼基底.自2007年11月至2011年10月,共治疗无明显骨性歪鼻的单侧唇裂鼻畸形成人患者25例.结果 本组25例患者,术后随访1～2年,外鼻形态持久,瘢痕不明显.结论 鼻翼软骨支架重组联合应用鼻中隔软骨移植,可以有效整复无明显骨性歪鼻的成人单侧唇裂鼻畸形.
Objective To evaluate the effect of nasal cartilages restructuring and septal cartilage grafting on repair of unilateral cleft lip combined with nasal deformity in adult. Methods An open approach was carried out to correct the deviation of the septal cartilage by means of repositioning of the caudal end to the midline and rotation sutures to realign the dorsal rim with the nasal upper lateral cartilage. The nasal tip was further reinforced with the columellar strut and shield grafts of the septal cartilage after realigning the nasal dorms of the bilateral lower lateral cartilages. The position of the nasal alar base was adjusted by local incisions and buried suspension sutures. Totally 25 patients of unilateral cleft lip without obvious bony deviated nose were brought into this study from November 2007 to October 2011. Results After 1 to 2 years follow-up, all the patients obtained persistent nasal shape with slight scars. Conclusion Nasal cartilage restructuring and septal cartilage grafting is an effective method for repairing the unilateral cleft lip combined with nasal deformity.
