摘要
目的:探讨高原条件下,烧伤休克复苏的方法。方法:模拟4 000 m 高原条件下,利用犬40%TBSAⅢ度烫伤模型,24 只犬被随机分为平原等渗组、平原高渗组、高原等渗组和高原高渗组,伤后1 h 开始补液。观察伤前、伤后2、6、12、24 h 和48 h 股动脉血压(FAP)、肺动脉血压(PAP)、肺动脉楔状压(PAWP)、肺血管阻力(PVR)、中心静脉压(CVP)、心输出量(CO)和心排指数(CI)等血流动力学参数及肺含水量的变化。结果:高原地区动物PAP、PVR明显高于平原地区,补液后高原等渗组PAP、PAWP、PVR和肺含水量显著高于高原高渗组,高原2 组之间FAP、CO和CI无显著差异。补液后平原2组之间各项指标无显著差异。结论:高原地区烧伤休克按平原地区常规方法复苏易引起肺水肿。
Objective: To evaluate the resuscitationaltechniques for burn shock under high altitude condition. Methods: 24 dogsw ere inflicted with 40% TBSA fullthickness scalding and random ized equally into norm al altitude isotonic fluid infusion (NI) group, norm alaltitude hypertonic fluid infusion (NH) group, high altitude (sim ulated 4 000 m ) isotonic fluid infusion (HI) group and high altitude hypertonicfluid infusion (HH)group. Fluid infusion began 1h postburn.Fem oralarterialpressure(FAP), pulmonary arterial pressure (PAR), pulm onary arterial wedge pressure (PAWP), pulm onary vascular resistance (PVR), centralvenous pressure(CVP), cardiacoutput(CO) and cardiacindex (CI) w ere measured in the 2nd, 6th, 12th, 24th and 48th hour postburn. Results: PAPand PVRwere significantly higherin the HI and HHgroupsthan in the NIand NH groups. PAP, PAWP, PVRand w atercontentoflung tissues were significant higher in the HIgroup than in the HH group but FAP, CO and CIshowed no significant difference between NIand NHgroups.Conclusion:When burn shock istreated in high altituderegions,the adm inistration ofhypertonic solution is ableto abatethe developm entofpulm onary edem a.
