庆大霉素重组合异种骨复合体防治开放性骨折骨缺损感染的实验研究 被引量:27

Experimental Study of Gentamicin-Reconstituted Bone Xenograft- Composite for Prophylaxis and Treatment of Bone Infections and Bone Defects After Open Fracture

摘要 目的研制庆大霉素重组合异种骨复合体(gentamicin-reconstituted bone xenograft-composite,G-RBX-C),为临床防治开放性骨折骨缺损感染提供理论依据。方法采用具有高效骨诱导及骨传导能力的重组合异种骨作为载体,复合庆大霉素制备庆大霉素重组合异种骨复合体,并对其体内药物释放特性、异位诱导成骨能力及骨缺损修复能力进行了研究。结果体内药物释放试验显示:G-RBX-C在 10天内持续释放高浓度的抗生素,12小时局部软组织中药物浓度可达 144.5ug/ml。血清药物浓度远低于庆大霉素的中毒浓度。异位诱导成骨试验显示:G-RBX-C植入2周即可见大量编织骨形成。修复兔桡骨节段性骨缺损试验表明:G-RBX-C植入4周可见多个新生软骨、骨岛,8周新骨大量形成,16周初步形成正常骨干结构,骨皮质连续,骨髓腔再通.结论G一RBX-C具有良好的体内缓释作用、异位诱导成骨能力及骨缺损修复能力,有望成为防治开放性骨折骨缺损感染的良好骨移植材料。 Objective To prepare gentamicin - reconstituted hone xenograft - composite(G -- RBX -- C) with the purpose of giving further information for its clinical application in prophylaxis and treatment of bone infections and bone defects after open fracture. Methods RBX as a bone grafting material which has the capabilities of osteoinduction and osteoconduction was used in combination with gentamicin to prepare a gentamicin -- RBX -- composite. The release test of gentamicin from these composites in vivo and its capabilities of ectopic bone induction and reparing segmental radial defects of rabbits were evaluated. Results in vivo release test of gentamicin revealed a high concentration level of antibiotics which sustained for at least ten days. In the study of ectopic bone induction, there was abundant woven bone in G -- RBX -- C group 2 weeks after the operation. The study of repairing segmental radial defects of rabbits showed abundant chondral and new bone islands at the site of the defects 4 weeks after operation, a number of new ossicles formed and increased in size by the sib week, and an early medullary canal appeared to be forming which contained marrow elements like substances by the 16th week. Conclussion G -- RBX -- C is of competent sustained release profile in vivo test and has the capabilities of ectopic bone induction and reparing large segmental nonunion defects. It might serve as a new hone grafting material in the prophylaxis and treatment of bone infections and bone defects after open fracture.
