摘要
目的 探讨妇幼保健院在降低区域内新生儿窒息发生率及病死率中的作用.方法 选择海淀区属4家分娩量最大的医院为研究对象,对在2009至2010年中开展新生儿窒息专项工作与2008年度未开展专项工作时的新生儿窒息发生率、早期新生儿死亡率进行比较.结果 4家医院新生儿窒息发生率2009至2010年比2008年均有逐年明显下降趋势,海淀区妇幼保健院、海淀医院、四季青医院和上地医院的新生儿窒息发生率,在各年度间比较差异有统计学意义（χ2分别为3.630、2.103、5.074、2.007,均P〈0.05）.4家医院合计早期新生儿死亡率2008至2010年分别为1.15‰、1.13‰、0.95‰,有逐年下降趋势,但各年度间比较差异无统计学意义（χ2=6.344,P〉0.05）.结论 妇幼保健院应承担起对区域内新生儿复苏工作的管理责任,规范的窒息复苏培训及督导、加强产儿科合作等是降低新生儿窒息发生率及早期死亡率的有力措施.
Objective To discuss the role of maternity and child health hospital in reducing the incidence and mortality of regional neonatal ＇asphyxia. Methods Four hospitals with the biggest childbirth quantity in Haidian district were selected as subjects. From 2009 to 2010 the neonatal asphyxia specific work was carried out in these 4 hospitals. The incidence of neonatal asphyxia and early neonatal mortality were compared with that in 2008 when this specific work was not carried out. Results The incidences of neonatal asphyxia of these 4 hospitals in 2009 and 2010 dropped significantly compared with that in 2008, and the difference in incidence of neonatal asphyxia of Haidian District Maternity and Child Health Hospital, Haidian hospital, Beijing Si Ji Qing Hopsital and Beijing Shangdi Hospital in same year was statistically significant （X2 was 3. 630, 2. 103, 5. 074 and 2. 007 respectively, all P 〈 0.05）. The total early neonatal mortality of 4 hospitals in 2008, 2009 and 2010 was 1.5‰, 1.13‰ and 0.95%0 respectively, indicating a dropping trend, but the difference in different years was not significant （X2 = 6. 344, P 〉 0. 05 ）. Conclusion Maternity and child health hospitals should assume management responsibility for the work of neonatal resuscitation in the jurisdiction areas. Standardized neonatal resuscitation training and supervision, and strengthening cooperation between pediatrics and obstetrics are effective means of reducing the incidence of neonatal asphyxia and early neonatal mortality.
