摘要
目前认为慢性扁桃体炎有3种学说,一是细菌感染,二是自身变态反应,三是免疫功能低下。一般认为发病多由急性扁桃体炎反复发作或隐窝引流不畅,窝内细菌或病毒滋生繁殖而演变为慢性炎症。近年来,国内外研究扁桃体隐窝上皮屏障功能,认为扁桃体隐窝上皮的屏障作用是决定扁桃体能否发生慢性扁桃体炎的重要原因,扁桃体隐窝上皮屏障功能的恢复,在慢性扁桃体炎的治疗中作用尤为重要。隐窝上皮是腭扁桃体中最先接触和摄取抗原,并引起免疫应答的部位,隐窝的特殊结构决定了扁桃体的功能主要是潴留过路的抗原,供免疫系统处置,因此隐窝上皮结构是扁桃体行使免疫功能的重要组织保障。慢性炎症病变使扁桃体隐窝上皮发生进行性破坏,使扁桃体产生的免疫球蛋白减少,因而不能发挥有效的免疫保护作用,所以任何治愈慢性扁桃体炎的保守措施,从组织学上首先必须恢复隐窝上皮的屏障结构。近几年来,有关慢性扁桃体炎的发病机制研究发展迅速,文献检索表明,现在还没有同类方法表明扁桃体隐窝上皮屏障功能改变对慢性扁桃体炎有影响的研究报道。文章就扁桃体隐窝上皮屏障功能作一综述。
Currently there are three theories about chronic tonsillitis：first,bacterial infection,and second,their allergy and the third is immunocompromised.It is generally believed that the incidence of recurrent acute tonsillitis by the poor drainage or crypt,nest breeding of bacteria or virus to evolve into chronic inflammation.In recent years,study abroad in the tonsil crypt epithelial barrier function found that the tonsil crypt epithelial barrier function is important to determine whether chronic tonsillitis occur.Tonsil crypt epithelial barrier function recovery and healing in the treatment of chronic tonsillitis are particularly important.Palatine tonsil crypt epithelium is the first contact and can intake of antigen,immune response and cause the site,and recess the special structure of the main function of the tonsils is crossing the retention of antigen,the immune system for the disposal,so the structure is the tonsil crypt epithelium immune function in the exercise of an important organizational guarantee.Chronic inflammatory disease makes tonsil crypt epithelium be destructed,so that the immune globulin reduced in tonsils,which can play an effective immune protection role,so any cure chronic tonsillitis conservative measures,from the histological crypt must first be restored epithelial barrier structure.In recent years,the pathogenesis of chronic tonsillitis rapid development of research,literature search shows that similar methods have not yet tonsil crypt epithelial barrier function to change the impact of chronic tonsillitis reported.In this paper,tonsil crypt epithelial barrier function is reviewed.
