摘要
目的通过运用动态血糖监测系统(CGMS)比较1型糖尿病患者持续皮下胰岛素注射(CSII)、一天多次胰岛素注射(MDI)、MDI联合阿卡波糖三种治疗方法的血糖控制情况。方法纳入30例1型糖尿病患者,分别予以CSII(CSII组)、MDI(MDI组)及MDI联合阿卡波糖(MDI联合阿卡波糖组)治疗,均佩戴CGMS 72 h,比较患者的高血糖和低血糖的时间及血糖波动参数。结果 CSII组糖化血红蛋白(HbA1c)水平(7.90±1.96)%明显低于MDI组(10.90±2.40)%,差异有统计学意义(P<0.05)。MDI组高血糖时间(585.83±139.26)min明显高于MDI联合阿卡波糖组(302.00±89.07)min,差异有统计学意义(P<0.05)。3组低血糖时间及反映血糖波动的指标包括血糖标准差(SDBG)、最大血糖波动幅度(LAGE)、平均血糖波动幅度(MAGE)及波动次数(NGE)比较,差异均无统计学意义(P>0.05)。结论 CSII治疗通过降低HbA1c更有利于1型糖尿病的血糖控制,联合阿卡波糖治疗有助于降低1型糖尿病患者的餐后血糖。
Objective To compare the efficacies of glycemic control in type 1 diabetic patients treated with continuous subcutaneous insulin infusion(CSII),multiple daily insulin injection(MDI),and MDI plus acarbose by using continuous glucose monitoring system(CGMS).Methods A total of 30 patients with type 1 diabetes receiving treatment with either CSII or MDI or MDI plus acarbose wore a CGMS sensor for 3 days to compare the general characteristics,duration and area of hyperglycemia and hypoglycemia,as well as the glucose variability parameters.Results The glycosylated hemoglobin(HbA1c) in CSII group was(7.90±1.96)%,and significant lower than(10.90±2.40)% in MDI group(P0.05).The duration of hyperglycemia in MDI group was(585.83±139.26)min,and obviously higher than(302.00±89.07)min in MDI plus acarbose group(P0.05).There was no significant difference among the three groups in duration of hypoglycemia and the indexes reflecting the glucose fluctuation such as standard deviation,largest amplitude of glucose excursion,mean amplitude of glucose excursion,and number of glucose excursion(P0.05).Conclusion CSII has beneficial effects on glyceamic control through the reduction of HbA1c.The use of acarbose in combination with insulin reduces postprandial plasma glucose levels in type 1 diabetic patients.
