摘要
目的探讨血液透析(hemodialysis,HD)患者发生心律失常的影响因素,以期为制定相应的防治策略提供资料。方法选取2010年4月至2011年1月在温岭市第一人民医院血液净化中心门诊行HD的患者为研究对象,统计其在HD过程中发生心律失常的次数。按照有无发生心律失常将患者分为心律失常组和非心律失常组,比较两组患者的一般情况及血液生化指标,并进行Logistic回归分析,探讨HD相关心律失常的危险因素。结果心律失常组患者的血红蛋白和HD次数低于无心律失常组,年龄和脱水量大于无心律失常组,差异均有统计学意义(均P<0.05)。两组患者在病程、HD前后血钾和血钙浓度、白蛋白和血肌酐等方面的差异均无统计学意义(均P>0.05)。Logistic回归分析显示,每次脱水量、HD次数和血红蛋白是HD相关心律失常的独立危险因素。结论 脱水量多、HD次数少和贫血较重是HD患者发生心律失常的影响因素。临床上必须对HD患者进行针对性的护理宣教,强化心律失常高危患者的护理,并完善抢救和护理措施,才能进一步提高护理质量。
Objective To evaluate the influencing factors of arrhythmia caused by hemodialysis（HD） so as to provide basis for establishing the corresponding prevention strategies.Methods From April 2010 to January 2011,84 HD patients in the hospital were enrolled in the study.The frequency of arrhythmia during the hemodialysis was recorded.Then the HD patients were divided into arrhythmia group and non-arrhythmia group.Comparisons were conducted on the general situation and blood biochemical parameters between the two groups.And Logistic regression analysis was used to determine the risk factors of arrhythmia caused by hemodialysis.Results The hemoglobin and the frequency of hemodialysis in the arrhythmia group were significantly less than those of the non-arrhythmia group（P〈0.05）,and the age and dewatering amount in the arrhythmia group were significantly higher than those of the non-arrhythmia group（P〈0.05）.However,no significant difference was detected on the course of disease,potassium and calcium concentration,albumin,and serum creatinine before and after HD（P〉0.05）.Logistic regression analysis showed that individual dewatering amount,HD number and hemoglobin were the independent risk factors related to arrhythmia.Conclusion Excessive amount of dewatering amount,less frequency of hemodialysis and anemia are the risk factors of arrhythmia caused by hemodialysis.To further improve nursing quality,HD patients must be given with clinically targeted nursing education,strengthened nursing of patients at high risk arrhythmias and a perfect rescue measures.
