温控射频消融仪在阵发性室上性心动过速射频消融中的应用 被引量:4

摘要 目的评价温度射频仪在阵发性室上性心动过速射频消融中的应用价值。方法对2443例阵发性室上性心动过速(PSVT)病人分别采用温度控制法(温控法)、功率控制法进行经导管射频消融,就其疗效及安全性进行了对比分析研究。结果2443例患者分为温控消融组和功率控制组,温控消融组共治疗1669例患者,成功率99.88%,功率控制组共治疗774例患者,成功率99.87%,两者无显著性差异。但温控组一次手术成功率明显高于功率控制组(93.4%﹠87.7%,P<0.01),功率控制组行二次消融手术的比例明显高于温控组(12.2%﹠6.4%,P<0.01);温控组平均放电时间及X-R曝光时间均小于功率控制组(P<0.05),具有统计学意义。温控组1例消融失败考虑旁道位于心外膜,1例为9岁儿童希氏束旁旁道,家属考虑风险放弃治疗。功率控制组1例房室结双径路患者在消融中出现Ⅲ度房室传导阻滞(Ⅲ0AVB)。结论应用温控射频仪采用温度控制法进行射频消融手术可以明显减少放电时间,减少了X-R的曝光量,增加手术成功率,减少严重并发症的发生,提高手术的疗效及安全性。 Objective To evaluate the application value of temperature controlled radiofrequency catheter ablation instrument for radiofrequency ablation(RF) of paroxysmal supraventricular tachycardia(PSVT).Methods Contrasting analysis of the efficacy and safety of temperature control method and specific energy consumption control method for RF of 2443 patients with PSVT.Results 2443 patients were divided into temperature control group(n=1669) and specific energy consumption control group(n=774).The success rate of temperature control group and specific energy consumption control group was respectively 99.88% and 99.87%,and no significant difference was observed between the two groups.The successful rate of the first operation in temperature control group was obviously higher than that in specific energy consumption control group(93.4%﹠87.7%,P0.01).The proportion of the repeat procedure in specific energy consumption control group was obviously higher than that in the temperature control group(12.2%﹠6.4%,P0.01).The average discharge time and X-ray time in temperature control group were less than that in specific energy consumption control group,indicating statistical significance(P0.05).In temperature control group,one case failed because the patient's accessory pathway was found under the visceral pericardium,and there was another aborted case with a 9-year-old patient with His bundle side pathway,whose family abandoned treatment given high risks;in specific energy consumption control group,one patient with dual atrioventricular nodal pathways developed third-degree atrioventricular block during ablation.Conclusion Radiofrequency ablation operation carried out through temperature control method with radio frequency ablation applied temperature controlled radiofrequency catheter ablation instrument can increase the efficacy and safety of the treatment,significantly decreasing discharge time and X-ray time,reducing the incidents of severe complications,and increasing the success rate of the o
