成人发育性髋关节脱位的全髋关节置换术

摘要 目的回顾分析成人发育性髋关节脱位患者的全髋关节置换术方法及手术疗效。方法对2005年6月至2010年6月接受THA的25例DDH患者进行随访。其中女19例,男6例,年龄35～60岁。采用Crowe分型:CroweI型11例11髋,CroweⅡ型9例9髋,CroweⅢ型4例4髋,CroweIV型1例1髋,并对CroweⅢ型和Ⅳ型常规进行CT扫描,了解关节及周围的骨量和解剖变异。Harris评分术前平均41.2。结果均随访4年,术后Harris评分优良率为92%,术后出现1例坐骨神经损伤患者,术后1年随访完全恢复。结论成人DDH的全髋关节置换手术难度较大,但对此类疾病来说是一种理想的治疗方法,提高了患者的生活质量。
