慢性胃病患者体内一氧化氮和内皮素的测定及意义

摘要 目的:探讨一氧化氮、内皮素在慢性胃炎、消化性溃疡中的病理生理意义及其与幽门螺杆菌感染的关系。方法:采用镉还原加显色法(格氏法)和放射免疫分析法测定一氧化氮(NO).内皮素(ET)水平,并用q检验及相关分析进行统计处理。结果:血浆、胃粘膜中NO水平:Hp阴性慢性胃炎(CG)组与正常组无显著性差异,而Hp阳性CG组则显著高于正常组(P<0.01,P<0.05)及Hp阴性CG组(P<0.01,P<0.05);Hp阴性消化性溃疡(PU)组显著低于正常组(P<0.01,P<0.05),但Hp阳性PU组则显著高于正常组(P<0.01,P<0.05)。血浆、胃粘膜中ET水平:Hp阴性、阳性CG、PU组均显著高于正常组(P<0.01);Hp阳性CG组显著高于Hp阴性CG组(P<0.01,P<0.05);Hp旧性PU组显著高于Hp阴性PU组(P<0.01,P<0.05)。正常组、Hp阴性PU组血浆NO与ET变化显著负相关(r=-0.672,r=-0.812;P均<0.05);其它各组无显著相关。结论:NO和ET可能参与了CG、PU的病理生理机制,并与Hp感染有关。 Objective To question the pathophysiologic meanings Of nitric oxide (NO), endothelin (ET) in chronic gastritis (CG) and peptic ulcer (PU) and the relations between them and helicobacter pylori(HP) infection. Method The plasma, gastric mucosa NO and ET were respectively determined with Cadmium-reduct plus Greiss' and RIA in 41 patients with CG, 32 patients with PU, 12 normal controls; and statistics treatment was made with q-test and relative analysis. Results The NO levels in plasma, gastric mucose in Hp-negative CG group had no significant difference with those in normal controls but the levels in Hp-positive CG group were significantly higher than thOSe in normal controls(P<0. 01. P< 0. 05 )and Hp-negative CG group(P< 0. 01. P< 0. 05 ); compered with normal controls. the levels in HP-negative PU group had significantly decreased (P< 0.01, P<0. 05 ) and the levels in.Hp-positive PU group had obviously increased(P< 0.01, P< 0. 05 ). The ET levels in plasma, gastric mucosa in each CG, PU group were all significantly higher than than those in normal controls (all P< 0. 01 ) ; the levels in Hp-positive CG group, PU group were respectively higher than those in Hpnegative CG group, PU group (P< 0. 01, P< 0. 05 ; P< 0. 01 , P< 0. 05 ). The content changes of plasma NO and ET in normal controls and Hp-negative PU group were significantly correlated negatively(r = - 0. 672, r = - 0. 812; P both <0. 05 ), but there was no aignificant relativity in the others. Conclusion NO and ET may be involved in the pathophysiologic mechanisms of CG and PU. Furthermore, it may be related to Hp-infection.
