摘要
目的分析原发性十二指肠癌的诊断及临床治疗措施。方法回顾性分析南阳市第二人民医院2006年7月至2011年7月诊治的24例原发性十二指肠癌的临床资料。结果该病以中老年男性多见;病变部位以十二指肠降部多见;病理类型以腺癌居多;临床表现以上腹痛、腹胀及上腹部不适多见;内镜及双源CT检查阳性率较高;胰十二指肠切除术16例(66.7%),3年生存率为32.5%。结论①原发性十二指肠癌早期症状不典型、无特异性,预后差;②内镜、十二指肠低张造影及双源CT检查联合应用,可互为补充、取长补短、提高诊断准确率;③提高对该病的认识和警惕性,做到早期诊断、早期治疗,可以提高远期生存率、改善预后。
Objective To examine the clinical diagnostic and therapeutic measures of primary du- odenal carcinoma（PDC）. Methods The clinical records of 24 patients with primary duodenal carcinoma during the period between July. 2006 and July. 2011 were retrospectively analyzed. Results Most of the patients were middle - aged or old aged men; The sites of primary duodenal carcinoma were more in descendant duodenum; The adenocarcinoma was more common in all pathologie types; The prominent clinical manifestations were abdominal pain, abdomen bulge and epigestria discomfort; The diagnosis rates of endoscopic and dual - source computed tomographic scan （DSCT） were more higher; 16 （66. 7% ） eases underwent pancreatoduodenectomy and their 3 -years survival rate was on- ly 32.5%. Conclusions （1）Primary duodenal carcinoma has its characteristic of no early characteristic manifestations, no particularity, and had prognosis; （2）Endoscopic, hypotonie duodenography and dual -source computed tomographie scan combinatively are additional relation and can learn from others＇ strong points to offset its weaknesses. Using these auxiliary examinations can help to achieve a high def- inite diagnosis rate; （3）To improve the understanding and attention of primary duodenal carcinoma and to achieve early diagnosis and treatment can improve the further survival rate and make the prognosis of the primary duodenal carcinoma better.
