摘要
目的 鉴于传统的肿瘤疗效评价体系存在的问题,分析现代肿瘤疗效评价体系的建立及其在医院管理中的作用方法 采取文献综述、专家咨询和核心小组讨论等方法结果 传统的肿瘤疗效评价体系存在的问题主要包括：①未体现新医学模式的整体观,②未体现对肿瘤本质的新认识（全身性）,③未体现以人为本的治＂人＂而非治＂病＂理念,④未体现现代人更重视＂活得好＂的积极心态.提出构建现代肿瘤疗效评价体系的设想和一些具体指标,包括疾病进展时间（TTP）、复发时间（MRT）、显效时间缓解期、无病生存期（DFSD）、病人报告结局/生命质量、临床受益疗效、疾病相关症状改善、经济（成本）/效果以及一些辅助指标/中间指标.提出应该从临床人员报告、医学检验（检查）报告、患者报告和照顾者（社会家庭）报告4个方面来构建现代的肿瘤疗效评价体系,其中前2个方面代表医疗方,主要基于传统的疗效标准,社会家庭方面主要考虑治疗的经济负担和临床受益,患者方面主要考虑生命质量或患者报告临床结局结论 构建现代肿瘤疗效评价体系对医院管理有重要的意义,有利于选择最优方案,减少医疗成本;有利于合理配置,有效利用医疗资源;有利于及时了解患者情况,减少不必要矛盾,缓解潜在医患纠纷;切合肿瘤专科医院医疗统计指标发展的需要,为相关指标的筛选提供参考.
Objective To analyze and outline the applications and contributions to hospital management of establishing the system of modem treatment effect assessment of tiamor. Methods Literature review,expert eonsuhing and focus group diseussion were used. Results The problems on traditional treatment effect assessment of tumor were discussed and thus the considering and some indicators to establish the system of modem one were presented. The system of treatment effect assessment of tumor should include indicators of four domains of doctor＇s reports, medical examinations reports, patients＇ reports and care-giver （society and family） reports. Of them, the two formers are mainly based on traditional treatments effects standards l while patients＇ reports include quality of life or patients＇ reports outcomes, care-giver reports focus on diseases burden and clinical benefits. Conclusion Establishing the system of modem treatment effect assessment of tumor is of important significance to hospital management. It is useful for rational allocation and effective usage of hospital resources, reducing medical costs by optional optimal treatments, and releasing possible medical disputes.
