摘要
目的:探讨过敏性紫癜(HSP)患者发病与幽门螺杆菌(HP)感染及链球菌感染的关系。方法:对94例过敏性紫癜患者进行14C尿素呼气试验检测幽门螺杆菌(HP),其中腹型紫癜54例,;非腹型40例,;另设30例健康儿童作为对照组。94例HSP中有46例同时检测了血清抗链球菌溶血素O(ASO),对照组40例。结果:腹型紫癜HP阳性42例(42/54,77.8%),非腹型HP阳性9例(9/40,20.25%),健康对照组HP阳性3例(3/30,10%);3组HP阳性率从高到低依次为腹型紫癜组、非腹型紫癜组、健康对照组,其差异有统计学意义(χ2=46.68,P<0.01)腹型紫癜组HP阳性率明显高于非腹型紫癜组及健康对照组(P<0.01);而非腹型紫癜组与对照组之间差异无统计学意义(χ2=1.886,P>0.05)。46例HSP患者ASO阳性9例,40例对照组ASO阳性5例,两组之间差异无统计学意义(P>0.5)。结论:幽门螺杆菌(HP)感染可能是腹型过敏性紫癜的诱因之一,抗HP治疗可减少腹型紫癜的反复,缩短病程;HSP的发生与A组溶血性链球菌感染的关系不大。
Objective：To investigate the relationship between Henoch-Sch nlein Purpura（HSP） in children and the infection of gastric Helicobacter Pylori（HP） and streptococcus.Methods：94 cases with Henoch-Sch nlein Purpura in childen were detected with 14C-Urea Breath Test（14C-UBT）,in which there were 54 cases with abdominal symptom,40 cases with non-abdominal symptom.There was 30 children for normal control group.Among those children with Henoch-Sch nlein Purpura,there were 46 cases whose serum anti-streptolysin＂0＂（ASO） was assayed and 40 children for normal control group.Results：Positive rate of HP was 77.8% in HSP group with abdominal symptom,non-abdominal symptom group was 20.25% and 10% for normal control group;The difference was significant（P0.01） between the abdominal symptom group and the non-abdominal symptom group.there was no significant difference between the non-abdominal symptom group and the normal control group（P0.05）.Among 46 children with HSP,there were 9 cases whose serum ASO was positive and 5 cases was positive for the normal control group and There was no significant difference between the two groups.Conclusions：HP infection may be one of the reasons of HSP with abdominal symptom in children and is not related with HSP with non-abdominal symptom.Treatment of HP can reduce relapse and course of HSP with abdominal symptom.The onset of HSP is not related with hemolytic streptococcus infection.
