摘要 目的探讨水劈核法在小切口非超乳软核白内障手术中的应用效果,旨在明确适应证,减少并发症,提高手术成功率。方法在47例(50眼)晶状体核Ⅲ级以下硬度小切口非超乳白内障手术中运用水劈核法处理软核。对劈核效果、手术相关并发症和复明效果进行观察。结果全部47例(50眼)中41眼顺利完成水劈核,占82%。完成水劈核41眼中术后角膜内皮水肿11眼,前房渗出反应3眼,并发症经处理后均消失;术后1周最佳矫正视力:0.1~0.3者2眼,0.3~0.5者5眼,≥0.5者34眼。结论根据软核白内障具有皮质较多较黏、晶状体核密度低的特点,选择水劈核法不失为一种可选择的、安全、有效、简便、合理、并发症少的手术方法,其适应证为Ⅲ级以下软核白内障。 Objective To investigate the efficacy of the application of the Hydrochop technique in the soft - nucleus small incision non - phacoemulsification, to define the indications, reduce the complications and increase the percentage of successful operations. Methods 50 eyes ( in 47 patients) under degree III hardness underwent the soft - nucleus small incision non -phacoemulsification cataract surgery with Hydrochop. Observations were conducted on the chop effect, surgery -related complications and vision regaining to evaluate the application of Hydrochop. Results Hydrochop was successfully completed in 41 eyes ( 82% ) , of which there was postoperative corneal edema in the endothelium in 11 eyes and anterior chamber exudation in 3 eyes. All the complications disappeared with management; by the end of first postoperative week, 2 eyes had a BCVA of 0.1 〈 0.3 ;5 eyes had a BCVA of 0.3 〈0.5; 34 eyes had a BCVA of≥ 0.5. Conclusion Due to the fact that soft - nuclear cataract is characteristic of having much cortex lentis and being glutinous, and low density in nucleus lentis, Hydrochop as an operation approach, is safe, effective, reasonable and convenient, with few complications and its indications are cases of soft - nucleus cataract below grade III.
