摘要
目的研究明胶海绵颗粒在TACE治疗巨块型原发性肝癌中对甲胎蛋白及转氨酶的影响。方法选取2009年1月至2011年6月进行治疗的80例巨块型原发性肝癌患者为研究对象,按栓塞方法不同将其分为对照组40例和观察组40例,对照组采用化疗药加碘化油混合乳剂进行栓塞治疗,而观察组则使用明胶海绵颗粒联合化疗药及碘化油混合乳剂进行栓塞治疗。将两组患者的AFP下降程度、转氨酶上升程度进行比较分析。结果观察组术后甲胎蛋白下降程度大于对照组(P<0.05),而术后转氨酶升高程度与对照组比较,差异无统计学意义(P>0.05)。结论与单纯用碘化油作为栓塞剂比较,在TACE中加用明胶海绵颗粒可增加原发性肝癌肿瘤标记物甲胎蛋白的下降程度,并且对肝功能的损害无加重。
Objective To investigate the effect of elatin sponge particle on AFP and the transaminase in the treatment of massive hepatocellular carcinoma. Methods The clinical data of 80 cases with massive hepatocellular carcinoma treated with endovascular embolization in my hospital from January 2009 to June 2011 were retrospectively analyzed, in which control group (n=40 cases) was treated with chemotherapeutic drug-lipiodol emulsion, and experimental group (n=40 cases) was embolized with chemotherapeutic drug- lipiodol emulsion and gelatin sponge particle. Statistical analysis was performed on the changes of the level of serum AFP and the transaminase level between two treatment groups after procedure. Results The changes of decrease in the serum AFP level were superior to that in control group expectively (P〉0.05), while no significant differences could be found for the change of transaminase level (P〉0.05). Conclusion In the treatment of massive hepatocellular carcinoma with endovascular embolization, compared to chemotherapeutic drug-lipiodol emulsion method, combined use of chemotherapeutic drug-lipiodol emulsion and gelatin sponge particle could promote the decrese of the serum AFP level and did not have more imoairment to the liver function.
