摘要
目的探讨髂骨原发恶性骨肿瘤的影像表现,以期进一步提高对该部位恶性骨肿瘤的认识及诊断水平。资料与方法回顾性分析47例经病理证实的髂骨原发性恶性肿瘤的影像表现。47例均行X线检查,22例行CT检查,37例行MRI检查。结果 47例中,多发骨髓瘤18例,淋巴瘤9例,软骨肉瘤8例,尤文肉瘤5例,骨肉瘤3例,恶性纤维组织细胞瘤、恶性骨母细胞瘤、纤维肉瘤及腺泡状软组织肉瘤各1例。病变主要表现为溶骨性骨质破坏,软组织肿块明显。多发骨髓瘤以"虫蚀"状破坏为主,淋巴瘤以"地图"状破坏为主。软骨肉瘤均见钙化,骨肉瘤均见瘤骨。结论髂骨原发恶性骨肿瘤的种类繁多、影像表现复杂。常见的原发性恶性骨肿瘤具有较特征性的影像表现,结合病变的临床及影像学表现,可提高病变的诊断准确率。
Objective To study the imaging appearances of primary malignant tumors in ilium,in order to improve the diagnostic accuracy.Materials and Methods The image appearances of 47 patients with primary malignant tumors in ilium were reviewed.All cases were confirmed by operation and pathology.All the cases underwent X-ray examinations,22 cases underwent CT and 37 cases underwent MRI examination.Results Of all the primary malignant tumors,multiple myeloma were found in 18 cases,lymphoma in 9 cases,chondroarcoma in 8 cases,Ewing＇s sarcoma in 5 cases,osteosarcoma in 3 cases,and malignant fibrous histiocytoma,malignant osteoblastoma,fibrosarcoma and alveolar soft tissue sarcoma were found in 1 case each.The major image features included huge soft masses and osteolytic bone destruction.The multiple myeloma showed as erode-like bony destruction,and the lymphoma showed as geographic shape bony destruction.There were calcification in chondroarcoma,and tumor bone in osteosarcoma.Conclusion Although the image features of primary malignant tumors in ilium are variety and complex,there are some major characters of the tumors.Combining the clinical and imaging can improve the diagnostic accuracy.
