摘要
目的:探讨以头痛为首发症状的急性闭角型青光眼的临床特点。方法:对52例以头痛为首发症状急性闭角型青光眼患者的临床资料进行分析,包括全身症状体征、眼部临床特点,首诊处理,眼科处理以及预后。结果:52例中,头痛伴有眼痛、恶心、呕吐31例,头痛、眼痛但无恶心、呕吐21例。首诊误诊为脑血管疾病者占81.54%,消化系统疾病者占15.38%。转眼科后43例行手术治疗,9例患者药物保守治疗,眼压均控制在<21mmHg。结论:急性闭角型青光眼患者常以头痛为首发症状首先就诊于内科,仔细进行眼部检查或请眼科会诊可以有效减少误诊,提高诊断的正确率。
Object：To explore the clinical features of 52 acuate angle - close glaucoma patients with fiirst symptom for headache patients. Method：To analysis the clinical features of 52 acuate angle - close glaucoma patients with fiirst symptom for headache patients, include the systemic and ocular symptoms and signs, the first patient treatment, eye treatment, and prognosis. Result ： First diagnosed by internal medicine for headache in all 52 patients. 31 cases in which with nausea and vomiting, and 21 ca- ses with no that. First misdiagnosed as cere- bral vascular disease in 81.54% , digestive disease in 15.38%, Referral for ophthalmol- ogy,43 cases received surgery,9 cases were treated by medicine,The IOP below 21 ram- Hg in all 52 eases. Conclusion：Many aeuate angle- close glaucoma patients with first symptom for headache patients were treated by internal medicine, Careful eye examination could decrease misdiagnosed rate and improve the diagnostic accuracy.
