Positioning the bronchus of pulmonary air leakage by injecting air into the bronchus and measuring pressure in the bronchus: A report of 19 cases

摘要 目的探讨Forgarty导管球囊充液封堵支气管,球囊前端支气管充气、测压定位气胸肺漏气支气管的方法、效果及安全性。方法在纤支镜直视下,将Forgarty导管沿纤支镜工作道插入到可疑靶支气管,采用球囊充液封堵靶支气管,球囊前端支气管充气、测压定位肺漏气支气管。结果 19例气胸患者中,顺利置入球囊导管、完成球囊充液、定位和封堵术16例,闭合性气胸定位失败3例。张力性和交通性气胸定位时间平均9 min;闭合性气胸平均15 min(P<0.05)。11例1次粘堵肺漏气支气管成功,4例2次,1例3次。定位过程中患者生命指征平稳,无明显不适。结论本气胸肺漏气支气管定位方法更加精准和具有较好的安全性,特别适合胸腔负压吸引时,引流管无明显气体溢出的气胸患者。 Objective To evaluate the efficacy and safety of a new method to determine the bronchus of pulmonary air leakage by injecting gas into the bronchus before the balloon and measuring the pressure in the bronchus.Methods The bronchus of pulmonary air leakage was detected by filling physiological saline into balloon of Frogarty catheter,blocking air-flow of airway,and injecting air and measuring the pressure into and in the bronchus before the balloon.Results Of 19 cases with pneumothorax,the bronchus of pulmonary air leakage was successfully positioned in 16 cases,only 3 failed on the closed pneumothorax.The average time of locating the bronchus of air-leakage was 9 min on the tensity and the transportation pneumothorax and 15 min on the closed pneumothorax(P0.05).Eleven cases achieved successful fibrin glue occlusion once,four cases twice and one case three times.All cases had stable life indicators and no obvious uncomfortableness in the process of positioning.Conclusion The new method is a simple,safe and more effective way to locate the broken bronchus of pneumothorax and especially suitable for the closed pneumothorax when no obvious gas flows through drainage tube.
