摘要
目的对新型TecnisZCB00一片式非球面人工晶状体(intraocular lens,IOL)和其他两种一片式非球面IOL在植入术后的眼内稳定性和全眼高阶像差(high order aberration,HOA)各成分及调制传递函数(modulation transfer function,MTF)值等情况作一比较,以了解这种新型非球面IOL设计特性对视觉质量的影响。方法对单纯性白内障患者116只眼进行术后3个月的随访。这些患者分别植入TecnisIOL(ZBC00)28只眼、AkreosIOL(AO)28只眼、Acysof SN60WFIOL(IQ)30只眼,以及AR40e IOL 30只眼进行iTrace和Pentacam检查。记录5mm瞳孔直径下的HOA各成分及MTF值,连同球面IOL组进行比较。使用Pentacam三维眼前房测量系统采集各眼Scheimpflug图像,用Image—pro plus6.0图像分析得出IOL的倾斜度和偏心量并进行比较。结果在水平和垂直方向上,各组患者IOL偏心值组间差异有统计学意义;在IOL倾斜度上,ZCB00组低于另两组但各组间差异无统计学意义。三种非球面IOL的高阶像差各成分比较,全眼及眼内高阶像差中只有球差差异均有统计学意义。除30c/d空间频率外,其他空间频率下不同非球面及球面IOL的MTF值差异均有统计学意义(P〈0.05o三种非球面IOL不同空间频率下各组MTF值差异无统计学意义。结论新型TecnisZCB00一片式非球面人工晶状体植入术后功能性视力明显提高,以襻部补偿设计为特征的3点固定可以增加囊袋内的稳定性,减少眼内IOL偏心倾斜的发毕.
Objective To measure tilt and decentration, modulation transfer function (MTF) of different kinds of aspheric intraocular lenses (IOLs) with Scheimpflug imaging systems after cataract surgery. Methods One hundred and sixteen eyes of patients for cataract surgery were offered 1-piece acrylic Tecnis IOLs (ZCB00) 28 eyes, 1-piece hydrophilic acrylic IOLs (Akreos AO) 28 eyes, 1-piece acrylic SN60WF IOLs (IQ) 30 eyes and 3-piece acrylic spheric IOLs (AR40e) 30 eyes. The MTF values and HOA of the IOLs with 5mm pupil were evaluated with iTrace examination. Measurements of IOLs tilt and decentration were obtained using Scheimphlug system (Pentacam, Oculus) and image-analysis with Image-pro plus 6.0 software. Results The vertical and horizontal decentration for all groups had different significantly. The IOL tilt measurements for ZCB00 group were lower, which showed no statistically significant differences. The MTF of the aspherical IOL groups were better than the other groups with all spatial frequency except 30c/d. Both aspheric IOLs provided good optics quality when compared with the spheric IOLs. Conclusions Functional visual acuity significantly increase after Tec- nis ZCB00 aspherical IOL implantation, its' unique three point loop design can increase the stability of the capsular bag.
