摘要
目的：探讨80岁以上高龄老人慢性心力衰竭病因及基础心脏病构成的特点。方法：按NYHA标准选取80岁以上高龄慢性心力衰竭患者306例，均符合心功能≥Ⅱ级心力衰竭，按发病机制分为左心室射血分数降低的心力衰竭（HFrEF）组75例和左心室射血分数正常的心力衰竭（HFnEF）组231例，每组按年龄又细分为80～89岁组（HFrEF组51例，HFnEF组170例）及90～98岁组（HfrEF组24例，HfnEF组61例），进行心功能、病因及基础心脏病构成的分析。结果：在HFnEF组不同年龄组中，女性患者明显多于男性（P〈O．05），心功能整体好于HFrEF组；HFrEF组中冠心病发病率明显高于高血压性心脏病（P〈0．01），而HFnEF组中高血压性心脏病发病率高于冠心病（P〈0．01）；高龄心力衰竭患者多种心脏病合并情况较普遍，单一心脏疾病患者较少（P〈0．01）。结论：高龄心力衰竭病因复杂，以多种心脏疾病伴随出现为突出特点，应将其作为一个特殊群体对待，临床中要多因素综合考虑，进行个体化治疗。
Objective： To discuss the etiology and characteristics of foundation heart disease constitution of elderly person over 80 years old with chronic heart failure. Methods： 306 cases over 80 years old patients who conformed to the heart function ≥ II Level heart failure according to the NYHA standard were selected. All cases were divided into HFrEF group which had 75 cases and HFnEF group which had 231 cases. According to the age, the two groups were divided into 80-89 years old group （HFrEF group had 51cases, HfnEF group had 170 cases） and 90-98 years old of group （HFrEF group had 24 cases, HfnEF group had 61cases）. Heart function, etiology and foundation heart disease constitute were analysed. Results： In different age group of HFnEF group, the female cases were more than male cases obviously （P〈0.05） and the heart function was better than HFrEF group. In HFrEF group, the morbidity of coronary heart disease was obviously higher than hypertensive heart disease （P〈0.01）, but in HFnEF group, the morbidity of hypertensive heart disease was higher than coronary heart disease （P〈0.01）. There was common in elderly patients with chronic heart failure combined with kinds of heart diseases, and patients with a single heart disease were less, the difference was Statistically significant （P〈0.01）. Conclusion： the reason of elderly heart failure is complex, and its prominent characteristic is followed with many kinds of heart diseases, elderly person over 80 years old with chronic heart failure should be taken as a special group to treat. Multi-factors should be considered in clinical, and take individuation treatment.
