摘要
目的:探讨中国南方人群桡、尺动脉内径特点及其在冠状动脉介入治疗中的临床意义。方法:入选2007-07至2010-09疑诊冠心病住院并首次经桡动脉途径行冠状动脉造影的患者1 346例,分为汉族组1 070例,壮族组276例,通过经桡动脉造影定量测量方法测量桡、尺动脉内径大小。结果:在1346例患者中,平均桡动脉内径为(3.04±0.43)mm,平均尺动脉内径为(3.03±0.37)mm,两者间对比差异无统计学意义(P>0.05)。86.1%男性及57.0%女性其桡动脉内径大于或等于6F Cordis鞘管外径;59.3%男性及24.9%女性其桡动脉内径大于或等于7F Cordis鞘管外径。汉族、壮族两组前臂动脉内径对比:桡动脉内径[汉族组(3.04±0.43)mm vs壮族组(3.05±0.42)mm,P>0.05],尺动脉内径[汉族组(3.03±0.38)mm vs壮族组(3.05±0.36)mm,P>0.05],差异均无统计学意义。多重线性回归分析显示桡动脉内径与性别(b'j=0.309,P<0.01)、体重(b'j=0.103,P<0.01)呈正相关,与糖尿病(b'j=-0.088,P<0.01)呈负相关。结论:中国南方汉族和壮族人群其桡、尺动脉内径大小差异无统计学意义。在经桡动脉途径操作中,大部分中国南方人群中使用6F鞘管及导管是安全的,在部分选择人群中使用7F鞘管及导管也是可行的。
Objective :To explore the inner diameter size of radial artery and ulnar artery with their clinical significance in percutaneous coronary intervention in patients of south China. Methods : A total of 1 346 patients with suspected coronary artery disease who received the first time trans radial coronary an- giography from July 2007 to September 2010 were summarized. The patients were divided into two groups: Han group, n= 1 070 and Zhuang group, n = 276. All patients' radial and ulnar artery inner diameters were measured by a computer assisted quantifica- tion method. Results:The mean radial artery inner diameter size was (3.04±0. 43)mm and the mean ulnar artery inner diameter size was (3.03 ±0. 37 ) mm,P〉0.05. There were 86. 1% of male patients and 57.0% of female patients with radial artery inner diameter ≥6F Cordies sheath; 59. 3% of male and 24. 9% of female patients with radial artery inner diameter ≥ 7F Cordis sheath. The inner diameters of radial artery in Han group and in Zhuang group were( 3.04±0.43 )mm vs. (3.05 ±0.42 )mm, P〉0. 05; the inner diameters of ulnar artery were (3.03 ±0. 38)mm vs. (3.05 ±0. 36)mm, P〉0.05. Multi regression analysis indicated that gender and body weight were positively related to the inner diameter size of radial artery ( bj = 0. 309, P〈0.01 ) and ( bjp = 0. 103, P〈0. 01 ) ; diabetes mellitus was negatively related to inner diameter size of radial artery( bji =-0. 088 ,P〈0. 01 ). Conclusion : In our work, Han group and Zhuang group in south China have the similar inner diameter size of radial artery and ulnar artery. It is safe to use 6F Cordies sheath with guiding catheter in transradial operation in most cases, and in some special cases,7F Cordies sheath with guiding catheter is feasible.
