摘要
目的探讨微创锁定钢板内固定治疗肱骨近端骨折的疗效。方法应用锁定钢板治疗肱骨近端NeerⅢ、Ⅳ型骨折62例,随机分为微创组30例,应用锁定钢板微创治疗;传统组32例,应用切开复位锁定钢板内固定。记录比较两组手术时间、术中出血量、切口长度、Neer评分及骨折愈合时间。结果微创组与传统组手术时间分别为(80.20±12.41)min和(122.50±20.20)min,术中出血量分别为(164.67±35.21)ml和(330.94±84.71)ml,切口长度分别为(9.07±1.05)cm和(15.56±1.24)cm,Neer评分分别为(90.40±7.96)分和(82.63±8.64)分。两组手术时间、术中出血量、切口长度、Neer评分比较差异均有统计学意义(P<0.05)。微创组与传统组骨折愈合时间分别为(18.53±3.22)周和(18.22±3.72)周,差异无统计学意义(P>0.05)。结论微创锁定钢板内固定治疗肱骨近端骨折能完成肱骨近端NeerⅢ、Ⅳ型骨折治疗,并获得满意效果。
Objective To observe the efficacy of the minimally invasive percutaneous plate osteosynthesis on the treatment of the proximal humerus fractures.Methods Sixty-two cases of Neer Ⅲ and Ⅳ proximal humeral fractures were treated with the locking proximal humerus plate.They were randomly divided into two groups: minimally invasive percutaneous plate osteosynthesis in 30 cases(minimally invasive group),and the traditional anteromedial incision approach in 32 cases(traditional group).All fractures were fixated with the locking proximal humerus plates.The operation time,the blood loss,the incision length,the postoperative Neer score,the healing time of bone were recorded.All the data were analyzed by SPSS 17.0 statistical software.Results The mean operative time was(80.20±12.41)min and(122.50±20.20)min in minimally invasive group and traditional group(P〈0.05).The blood loss was(164.67±35.21)ml and(330.94±84.71)ml(P〈0.05).The incision lengths were(9.07±1.05)cm and(15.56±1.24)cm(P〈0.05),and the Neerscores were(90.40±7.96) and(82.63±8.64) in minimally invasive group and raditional group(P〈0.05).The fracture healing time was(18.53±3.22)week and(18.22±3.72)week in minimally invasive group and traditional group.There was no statistical difference(P〉0.05).Conclusion The treatment of Neer Ⅲ and Ⅳ proximal humeral fracture with the minimally invasive percutaneous plate osteosynthesis is a perfectly innovational method.
