摘要
缺血性脑卒中是脑血管病最常见的类型,其最主要的病因是高血压。首次卒中后存活的患者后6个月内是卒中复发危险性最高的阶段,第1年内的复发数约占复发总数的30%,所以有必要尽早开展二级预防。既往二级预防中常用降压药联合阿期匹林预防卒中的复发,但阿期匹林有47%的人存在用药抵抗。我科自2007年1月始进行中成药天欣泰血栓心脉宁片联合缬沙坦对高血压性缺血性脑卒中的复发进行二级预防的研究。将卒中后存活的病例随机分两组,缬沙坦联合阿期匹林为对照组,经观察治疗18个月,两组在脑梗死复发率上无明显统计学差异(P>0.05)。本研究结果显示天欣泰血栓心脉宁通过活血化瘀、降脂抗凝,缬沙坦通过平稳降压,稳定脑动粥样硬化的易损斑块,确能显著降低高血压性缺血性脑卒中的复发率,且副作用少,老年患者更能耐受,依从性更好,值得在临床上进一步研究。
Ischemic stroke is the common syndrome of cerebrovascular disease,the main reason is hypertension.The first,the last 6 month was the stage of highest risk stage on the patients who lived after stroke,the number of relapse during the first year account for 30%of total recurrence number,so we had necessary to carry out secondary prevention.Previous secondary prevention also using antihypertensive drug plus aspirin to prevent recurrent of stroke,47%of patients had drug resistance on aspirin.My hospital made secondary prevention study on preventing recurrence of hypertensive ischemic stroke with Tianxin Tai thrombus Xinmai Ning plus valsartan since January 2007.Divide the patients who lived after stroke into two group,valsartan plus aspirin for control group,after treating for 18 months,recurrence rate of cerebral infarction had no statistically significant difference（P0.05）.The results indicated that Tianxin Tai thrombus Xinmai Ning can Huoxue Huayu,Jiangzhi Kangning,Valsartan can stabilize cerebral atherosclerosis vulnerable plaque according to stable buck,reduce recurrence rate of hypertension,ischemic stroke,and side effects was less,better tolerated in elderly patients,better compliance,worthy of further study in clinical.
