摘要 目的研究一氧化氮(NO)在大鼠任意型皮瓣中的作用,并探讨NO介导的炎症因子IL-6和TNF-α对任意型皮瓣存活的影响,为临床皮瓣的治疗提供有效途径。方法建立任意型皮瓣大鼠模型。按照动物随机数字表将56只Wistar大鼠随机分为左旋精氨酸组(L-Arg组)、实验组(T组)和对照组(C组)。分别应用硫代巴比妥酸法检测皮瓣组织丙二醛(MDA)的含量,应用黄嘌呤氧化酶法检测超氧化物歧化酶(SOD)的活性;应用双抗体夹心酶联免疫(ELISA)法检测大鼠血清IL-6和TNF-α水平;应用皮瓣存活长度比皮瓣实际长度计算出皮瓣存活率。结果①与C组比较,L-Arg组和T组大鼠皮瓣掀起后MDA含量升高,SOD活性下降;IL-6和TNF-α水平升高。②皮瓣掀起第1、3、7天,与T组比较,L-Arg组皮瓣组织MDA含量显著下降(P<0.01),SOD活性显著升高(P<0.01),血清中IL-6和TNF-α水平明显降低(P<0.01)。③皮瓣掀起第7天,与T组比较,L-Arg组皮瓣成活率明显升高(P<0.01)。结论①皮瓣移植术后已经存在氧化应激和炎症反应损害,表现为MDA含量升高,SOD活性下降,IL-6和TNF-α水平升高。②NO介导炎症因子的降低可以改善氧化应激状态,减轻炎症反应,对皮瓣移植具有保护作用。 Objective To study the effect of nitric xoide(NO) in rats of random patterns skin flap and explore the influence of NO to the survival of the random patterns skin flap by mediating the inflammatory factor of IL-6 and TNF-α and provide an effective way to treat the survival of skin flap in clinical.Methods Established models of random patterns skin flap in rats.56 Wistar rats were divided into three groups randomly:①L-arginine(L-Arg) group,models of random patterns skin flap and treated by L-Arg;②T group,models of random patterns skin flap and treated by sodium chloride;③C group,normal rats treated by sodium chloride.The content of MDA in tissue of skin flap was detected by thiobarbituric acid;the activity of SOD in tissue of skin flap was detected by xanthine oxidase;the level of IL-6 and TNF-α in blood serum was detected by ELISA;survival rate of skin flap was the average survival length of skin flap deviding practical length of skin flap.Results ①Compared with C group,the content of MDA in the uncover skin flap of T and L-Arg group increased(P0.01),the activity of SOD significantly lower(P0.01),the level of IL-6 and TNF-α in blood serum of T and L-Arg group increased(P0.01).②The skin flap uncovered by the first day,the third day,the seventh day,compared with T group,the content of MDA in the skin flap of L-Arg group lower(P0.01),the activity of SOD significantly increased(P0.01),the level of IL-6 and TNF-α in blood serum lower(P0.01).③The skin flap uncovered by the seventh day,compared with T group,survival rate of skin flap of L-Arg group significantly higher(P0.01).Conclusion ①The uncovered skin flap exists damage of the oxidative stress and inflammatory reaction,including:the content of MDA increased,the activity of SOD lower,the level of IL-6 and TNF-α increased.②NO mediating the degradation of inflammatory factor can improve the state of oxidative stess and lighten inflammatory reaction to protect the survival rate of skin flap.
