摘要
目的研究口腔颌面肿瘤患者外周血细胞免疫的变化及临床意义。方法采用流式细胞术(FCM)对30例正常人、30例口腔颌面良性肿瘤及60例口腔颌面恶性肿瘤患者的外周血CD3、CD4、CD8、CD56、CD4/CD8进行定量检测。结果口腔颌面恶性肿瘤患者外周血CD3、CD4、CD4/CD8均显著性低于对照组及良性肿瘤组(P<0.05);而CD8和CD56则上升(P<0.05)。良性肿瘤患者外周血CD3、CD4、CD4/CD8较健康人降低(P>0.05),而CD8和CD56较健康人升高(P>0.05)。恶性肿瘤患者中有淋巴结转移组的CD3、CD4、CD4/CD8较无淋巴结转移组降低(P<0.05),而CD8和CD56则升高(P<0.05)。临床分期Ⅲ、Ⅳ组的CD3、CD4、CD4/CD8较临床分期Ⅰ、Ⅱ组降低(P<0.05),而CD8和CD56则升高(P<0.05)。结论口腔颌面恶性肿瘤患者存在细胞免疫功能降低,且其免疫抑制远较良性肿瘤及健康人群强;有淋巴结转移的患者和临床分期晚期患者免疫抑制较无淋巴结转移者和临床分期早期患者显著;在临床中动态地观察各项细胞免疫指标,对判断病情,疗效和预后及提供口腔颌面恶性肿瘤的免疫治疗依据都具有一定的意义。
Objective To investigate immune function of the cell in the peripheral blood and its meaning for the clinical practice.Methods Using flow cytometry and monoclonal antibodies to evaluate CD3、CD4、CD8、CD56、CD4/CD8 T cells of peripheral blood in 30 healthy people,60 patients with oral cancer and 30 patients with oral benign tumor.Results The CD3、CD4 T cells and helper/suppressor radio（CD4/CD8） were significantly less（P0.05）,and CD8、CD56 T cells were significantly more（P0.05） in patients with oral cancer than those in patients with oral benign tumor and normal controls.The CD3、CD4、CD8、CD56 T cells and helper/suppressor radio（CD4/CD8） had no significant difference in patients with oral benign tumor amornt of with the normal controls（P0.05）.The CD3、CD4、CD8、CD56、CD4/CD8 were correlated with lymph node metastasis and clinical stages（P0.05）.Conclusions Oral cancer has an obvious immune suppressive function.The depression of immune function is much more obvious in the patients who suffer from oral cancer than those who suffer from oral benign tumor and nomals.The depression of immune function was correlated with lymph node metastasis and clinical stage.It must be very useful in the clinical practices to evaluate the immuocyte in peripheral blood in order to judge the disease and its curative effect,prognosis,and it can also provide a basis for the immunotherapy of oral cancer.
