摘要
目的通过对胸腔积液中的端粒酶活性、胸液及血清HER-2蛋白浓度联合检测,探讨它们在胸腔积液性质鉴别诊断中的价值。方法抽取87例恶性胸腔积液患者血液及胸腔积液后采用反转录-聚合酶链反应(RT-PCR)检测胸液端粒酶活性,酶联免疫吸附试验(ELISA法)检测血清、胸液HER-2蛋白的浓度,检测同期35例良性胸腔积液作对照。结果恶性胸腔积液患者胸液中端粒酶活性、HER-2蛋白浓度、血清HER-2蛋白浓度较良性胸腔积液患者显著增高(P均<0.05);胸液中HER-2蛋白诊断恶性胸腔积液敏感性为85%,特异性为91%,正确率为88%,端粒酶诊断恶性胸腔积液敏感性为88%,特异性为90%,正确率90%;血清HER-2蛋白诊断恶性胸腔积液中敏感性为84%,特异性为92%,正确率为85%;胸液中端粒酶、HER-2蛋白、血清HER-2蛋白联合检测的敏感性为67%,特异性98%,正确率为79%。结论端粒酶、HER-2蛋白检测对良恶性胸腔积液的鉴别诊断有一定的临床意义,联合检测端粒酶、HER-2对恶性胸腔积液诊断具有较高价值。
Objective It is to approach the diagnostic value of combining detection of telomerase activity and HER-2 protein in differentiating malignant from nonmalignant pleural effusion.Methods Eighty-seven specimens of malignant pleural effusion and thirty-five specimens of nonmalignant pleural effusion were examined for telomerase activity and HER-2 protein level by PCR and ELISA respectively.Results The level of telomerase and HER-2 protein in malignant pleural effusion was significantly higher than that in benign one.The serum level of HER-2 protein in patients with malignant pleural effusion was significantly higher than that in patients with benign one,too.The diagnostic efficiency of telomerase activity in pleural effusion as following： sensitivity 88%,specificity 90%,accuracy 90%;HER-2 protein in pleural effusion as following： sensitiviracy 85%,specificity 91%,accuracy 88%;serum HER-2 protein as following： sensitivity 84%,specificity 92%,accuracy 85%.The sensitivity,specificity,and accuracy of combined detection were 67%,98%,and 79%,respectively.Conclusion Telomerase activity and HER-2 protein have determinate diagnostic value in differentiating benign from malignant pleural effusion.Diagnostic rates of differentiating benign from malignant will be elevated by detection of combined telomerase activity with HER-2 protein.
