摘要 目的应用脉冲振荡法(impulse oscillometry,IOS)测定慢性阻塞性肺疾病(chronic obstructive lung dis-ease,COPD)患者的呼吸阻抗,并与常规肺功能进行比较,探讨其临床应用价值。方法对172例COPD患者和60例健康对照者进行IOS测定与常规肺功能测定,测定指标进行比较分析。结果 COPD各组与对照组比较,Fres、Z5、R5、R20、R5-R20、Rp明显升高(P<0.05),Fres、Z5、R5、R5-R20、Rp随病情加重而逐渐升高,R5-R20在COPD各组间比较具有显著性差异(P<0.05)。COPD各组与对照组比较,X5则明显下降,具有显著性差异(P<0.05)。IOS输出图形具有显著特点。Fres与FEV1/pre%、FEV1/FVC%相关性最强,相关系数分别为-0.65、-0.64。结论 IOS检测可反映COPD患者气道阻塞程度、评估其气道阻塞部位及严重程度,与常规肺功能检测结合可为COPD诊断提供依据,具有重要的的临床应用价值。Fres是IOS肺功能检查中诊断COPD较为敏感的指标。 Objective: To explore the clinical application value of IOS by testing respiratory resistance of patients with COPD and comparing the indexes with that tested by routine pulmonary functions testing.Methods: The IOS and routine pulmonary functions parameters of 172 patients with COPD were measured and compared with that of 60 healthy people.Results: Compared with that of the control group,respiratory impedance(Z5),respiratory resistance at 5Hz(R5),R5-R20,and peripheral resistance(Rp) in patients of COPD group increased significantly.And with the state of an illness being aggravated,Fres、Z5、R5、R5-R20、Rp are increased gradually.R5-R20 have significant difference among COPD groups.Compared with that of the control group,X5 decreased significantly in COPD groups,and the difference is significant.IOS output figures have significant characteristic.Fres have most significant correlation with FEV1/pre%、FEV1/FVC% and the correlation coefficient are-0.65 and-0.64 respectively.Conclusion: IOS testing could not only reflect the airway obstruction degree,but also be used to evaluate the airway obstruction location and severity degree.In combination with routine pulmonary functions testing,IOS testing could present strong evidence for diagnosis of COPD.IOS testing had higher clinical application value.Fres is a sensitive index for diagnosis of COPD.
