摘要
目的:检测不同病变程度冠心病患者外周血单个核细胞NF-κB的表达,探讨NF-κB与冠脉病变的关系及其临床意义。方法:经冠状动脉造影证实的97例急性冠脉综合征(ACS)患者和56例稳定型心绞痛(SAP)患者,另外选择冠脉造影正常的患者50例为对照组。分别在入院后采集肘静脉血液标本,采用流式细胞仪测定外周血单个核细胞NF-κB的表达活性,结合改进的冠脉危险积分法评价冠脉病变情况。结果:各组患者性别、年龄、总胆固醇、高密度脂蛋白间无差异,但ACS组糖尿病者的比例及LDL-C水平均显著高于对照组和SAP组(P<0.05 or P<0.01);ACS患者改进冠脉危险积分、hs-CRP、NF-κB水平亦显著高于SAP组和对照组(P<0.05),SAP组hs-CRP、NF-κB水平与对照组比较差异也有显著性(P<0.05),改进的冠脉危险积分与hs-CRP、NF-κB水平有明显的相关性(P<0.01)。结论:不同病变程度冠心病患者NF-κB的水平不同,NF-κB可能参与了冠心病的病理生理过程,它的高表达提示患者可能存在着严重的冠脉狭窄。
Objective: To detect the expression of factor kappa B in the peripheral blood and its correlation with the patients of stable angina pectoris(SAP) or acute coronary system(ACS).Methods: The 97 patients with ACS,56 patients with SAP and 50 patients without coronary artery disease(control group) were studied.Blood samples were collected from ulnar vein after the patients were in hospital,the expression of NF-κB activation of PBNC was detected quantificationally by flow cytometer with the NF-κB Kit.The improved jeopardy score for coronary artery lesion was evaluated according to the method introduced by documents.Results: These is no difference in ages,sex,total cholesterol(TC) and high density lipoprotein cholesterol(HDL).The level of low density lipoprotein cholesterol(LDL) and diabetic in ACS group are much higher than these in SAP group and Control group(P0.05 or P0.01).The expression of factor kappa-B,improved jeopardy score and hs-CRP in ACS group were higher than those in control group(P 0.05).There was significant difference in NF-κB,hs-CRP between SAP and ACS groups(P0.05).The expression of NF-κB were positively correlated with the improved jeopardy scores(P0.01).Conclusion: The expression of kappa-B in the peripheral blood may play a key role in the progression of coronary artery disease,high NF-κB show that there is serious lesion in the coronary artery.
