下胸椎微创徒手椎弓根置钉技术的初步研究 被引量:2

Preliminary Study of Free Hand Pedicle Screw Replacement Technology in Lower Thoracic Spine under Minimally Invasive

摘要 目的:探讨微创徒手椎弓根置钉技术在下胸椎中应用的准确性研究。方法:2007年3月~2010年4月选定A组17例不需椎管减压胸腰段椎体骨折患者,术前CT三维重建及定位钢丝网辅助拍片确定椎弓根体表投影,做2cm切口,单纯依靠手感行徒手椎弓根钉置入,常规行骨折复位固定术。术后CT评价40枚下胸椎螺钉置入位置,统计相关的螺钉置入情况及并发症发生率,并与B组同期18例切开置入的40枚下胸椎椎弓根钉相比较。结果:A、B两组神经血管及内脏并发症发生率均为0,螺钉置入情况比较差异无统计学意义(P〉0.05)。随访中未发现断钉、断棒及拔钉情况。结论:下胸椎微创徒手椎弓根置钉技术安全可靠,有诸多优点适合临床推广。 Objective:To detect the accuracy application of free hand pedicle screw replacement technology in lower thoracic spine under minimally invasive.Methods:Seventeen cases of A group with thoracolumbar fractures but without decompression from March 2007 to April 2010,using three-dimensional reconstruction of CT and positioning steel wire X-ray to determine the surface projection of the pedicle of vertebral arch.Then made 2 cm incision,free hand inserted pedicle screw relying solely on hand-line,routine fracture reduction and fixation.The position of 40 cases of thoracic screw placement was evaluated by CT after operation.While,the relative screw placement and complication rate were statistic,and which were compared to 40 cases of thoracic screw of B group.Results:There was no blood vessels and visceral complication in two groups,and there was no significant difference in thoracic screw between the two groups(P0.05).There were no broken nails,broken rods,and screw off.Conclusion: Free hand pedicle screw minimally invasive replacement technology is secure and there are many advantages for clinical application.
