摘要
目的分析生物型人工全髋关节置换治疗强直性脊柱炎(AS)的早、中期疗效。方法自2003年6月~2010年6月采用生物型假体治疗12例24髋强直性脊柱炎患者。临床评估以Harris评分为标准,影像评估根据置换后随访双髋关节正、侧位X射线片,观察臼杯、股骨假体位置及周围骨质改变,并测量臼杯内衬磨损速度与趋势。结果 12例均获随访,随访1~5年(平均3年6个月),Harris评分由术前(40.3±3.3)分恢复到术后(94.5±3.8)分。假体均位于解剖位置,尚未见术后感染及松动发生,患者双下肢长度差术前为1~5cm,术后为0~1cm,11例22髋疼痛完全消失,1例2髋有轻度疼痛伴有轻度跛行,但能够生活自理且恢复正常工作,其余患者均步态正常。结论生物型假体用于治疗强直性脊柱炎晚期患者,能够提供理想固定的效果,临床疗效满意。
Objective To explore the efficacy of biological-type prosthesis in the treatment of ankylosing spondylitis.Methods From June 2003 to June 2010,we used biotype total hip replacement to treat 12 cases of ankylosing spondylitis (24 hips).Taking Harris score as standards to assess clinical efficacy,while image assessment based on positive and lateral X-ray film at the time of follow-up,observed the acetabular cup and femoral component positions around the bone changes,and measured the acetabular cup liner wear rate and trends.Results All the 12 cases gained a follow-up of 1 to 5 years.Harris score improved from (41.5±2.3) points before operaion to (93.2±3.5) points postoperatively.The implants were all located in the anatomical position without infection and loosening.The preoperative leg length difference was 1 to 5 cm while 0 to 1 cm postoperatively,11 cases (22 hips) achieved complete pain remission and regained normal gait,while 1 patients (2 hips) resulted in mild pain with slight limp,but could return to normal daily living and work.Conclusion Biological prosthesis can provide the ideal fixed effect in the treatment of ankylosing spondylitis at later stage,with satisfactory early and midterm clinical results.
