摘要
目的探讨高血压患者血清补体C3、超敏C反应蛋白(hs-CRP)水平变化与胰岛素抵抗(IR)指数(HOMA2-IR)的相关性。方法选择2008~2010年重庆医科大学附属第一医院内分泌科年龄25~80岁的高血压患者139例,以HOMA2-IR上四分位为切点分为IR组与非IR组[HOMA2-IR≥1.47为IR组(n=35);HOMA2-IR<1.47为非IR组(n=104)],观察两组患者补体C3、hs-CRP水平变化与HOMA2-IR的相关性。结果 IR组血清补体C3和hs-CRP水平显著高于非IR组。校正性别、年龄、BMI、腰围后,补体C3仍与HOMA2-IR正相关(P<0.05)。多元线性逐步回归分析表明,补体C3是HOMA2-IR的主要影响因素(P<0.05)。结论在高血压人群中,补体C3独立于肥胖与HOMA2-IR相关,且相关性强于hs-CRP,补体C3可能是反应IR炎症状态更好的生物学指标。
Objective To observe the serum levels of complement component 3(C3) and high-sensitive C-reactive protein(hs-CRP),and to compare their association with HOMA2-IR in hypertensive patients.Methods 139 hypertensive subjects were recruited.Weight,height,blood pressure,wasist circumference,fasting plasma glucose(FPG),fasting serum insulin(Fins),blood lipids,C3 and hs-CRP were measared.HOMA index(HOMA2-IR) was calculated.Insulin resistance was defined as the upper quartile of HOMA2-IR.Results C3 and hs-CRP were significantly higher in subjects with IR compared with subjects without.After adjustment for gender,age,BMI,and waist circumference,C3 was still positively and significantly correlated with HOMA2-IR(P0.05).By regression analysis,C3 was the main influential factor of HOMA2-IR(P0.05)Moreover,compared with low C3 subjects,high C3 subjects were 3.54(P0.01)times more likely to be with IR.Conclusion Compared to hs-CRP,C3 has a stronger association with HOMA2-IR,independent of obesity;C3 might be a better inflammatory marker of insulin resistance in hypertensive subjects.
