摘要
目的:探讨彩色多普勒血流显像(CDFI)诊断右室双出口及并发心内畸形的价值。方法:收集右室双出口25例行多切面二维超声心动图彩色多普勒检测,按Van Praagh分段法对右室双出口心脏结构进行诊断,分析超声图像表现。结果:大动脉并列关系有6例,正常方位13例,右异位6例。CDFI检出心内并存畸形,如室间隔缺损、室间隔多发缺损、右室流出道梗阻、动脉导管未闭、房间隔缺损、完全型房室隔缺损、二尖瓣闭锁、完全性大动脉转位、肺动脉闭锁、二叶式肺动脉瓣、肺动脉瓣狭窄、房间隔膨出瘤并缺损、右房异构、永存左上腔、镜像右位心、右位主动脉弓、主动脉弓离断(B型)、左无名静脉低位、侧枝血管形成、左室发育不良等。结论:多切面二维超声心动图和CDFI技术相结合无创伤性诊断右室双出口及其并发心内畸形具有重要价值,可作为首选或筛选方法。
Objective：To evaluate the value of color Doppler flow imaging（CDFI） in diagnosing double outlet of the right Ventricle（DORV） and detecting additional complicated intracardiac abnormalities.Methods：Collecting 25 cases of DORV with two-dimensional multi-slice color Doppler echocardiography,according to Van Praaph＇s method in DORV of the heart structure of diagnosis,analysis of the performance of ultrasound images.Results：6 cases with juxtaposition of two arraies,13 cases of normal position,and 6 cases of edxtro-malposition.CDFI detected the coexistence of heart malformation,such as VSD,Multiple VSD,Right ventricular outflow tract obstruction,PDA,ASD,CAVC,MA,TGA,pulmonary atresia,Bicuspid pulmonary valve type,PVS,Atrial septal aneurysm expansion and defect,right atrial isomerism,LSVC,mirror dextrocardia,AA,IAA（B）,left innominate vein low,collateral vessel formation,left ventricular hypoplasia.Conclusion：Multi-slice two-dimensional echocardiography and CDFI technology combined with non-invasive diagnosis of double outlet right ventricle and the complicated heart malformation is of great value.It can be the first choice of screening method.
